Objective To investigate the clinic effect of the operative management of displaced intra-articular glenoid fractures. Methods 22 case of displaced intra-articular glenoid fractures were treated with open reduction and internal fixation with contoured reconstructive plate by means of modified Judet approach and partly anterior approach. The patients' recovery of shoulder joint surgical and functional outcomes after operation, the postoperative complications,the bone healing were recorded. Results All the patients wound healed without infections. The 18 patients were follow-up for 12 to 39 months (average time 16 months). All the scapular fractures united uneventfully without internal fixation loosening and the reseted articular facets didn't take place obvious displacement. According to shoulder joint functional evaluation standard: 14 patients had excellent results,3 fine,l satisfied,and 0 unsatisfied. Conclusion Good clinical outcomes can be obtained when open reduction and internal fixation with reconstructive plates are selected to treat the displaced intra-articular glenoid fractures.%目的 探讨手术治疗肩胛盂移位性关节内骨折的临床疗效.方法 采用改良Jdet入路为主的多种组合手术入路,重建钢板内固定治疗22例肩胛盂移位性关节内骨折,对患者术后肩关节功能恢复情况进行随访,并记录患者术后并发症、骨折愈合情况和肩关节前屈、内旋、内收及外展功能.结果 所有患者术后均未出现切口感染.有18例患者得到随访,随访时间12～39个月,平均16个月,1例随访患者X线片显示创伤性关节炎,其余患者X线片下肩胛骨骨折骨性愈合良好,复位关节面平整未发生明显移位,内固定物无松动及脱出现象.根据Schandelmaier等肩关节功能标准评定,优14例,良3例,可1例.结论 对于肩胛盂移位性关节内骨折行重建钢板内固定治疗可获得良好的临床效果.
