Objective To investigate the venous reflux reconstruction of the amputated finger without venous anastomosis, improve the survival rate of replantation of amputated finger without venous anastomosis. Methods The amputated fingertips in 40 cases (52 lesions)were replanted with a venous drainage through medullary cavity by 2.0 mm diameter K-wires with depth of 3.0~6.0 mm from July 2008 to June 2013. Results 36 cases (46 fingers) were survived after the operation. They were followed up for 3 to 18 months. The length of the survival fingers were roughly the same with the contralateral fingers, and with the normal growth of finger nails, ruddy skin. Nearly normal appearance and satisfactory sensation of the distal phalanx were obtained with 2-PD of 3.0~6.0 mm. Conclusion A venous drainage through medullary cavity can establish venous drainage of the amputated finger without venous anastomosis, improve the survival rate of replantation and with good clinical effect.%目的 探讨应用扩髓方法重建无静脉可供吻合断指再植的静脉回流. 方法 2008年7月—2013年6月,采用直径2.0 mm的钻头将断指近远断端扩大髓腔,深度为3.0~6.0 mm的方法再植无静脉可供吻合的断指40例52指. 结果 术后成活36例46指,随访3~18个月,成活手指长度与健侧指大致相同,指甲正常生长,指端皮肤红润,痛温觉恢复,两点分辨觉可达3.0~6.0 mm. 结论 采用扩髓方法重建无静脉可供吻合断指再植的静脉回流,能有效提高再植成活率,临床疗效良好.
