指动脉终末背侧穿支血管链逆行岛状皮瓣修复指端缺损

摘要

Objective To estimate the curative effect of repairing fingertip defects with reverse island flap pedicled with terminal dorsal branch of digital artery. Methods 20 patientsof (25 fingers) fingertip defects were treated by reverse island flap pedicled with terminal dorsal branch of digital artery of the same finger. Results The flaps survived in 19 patients (24 fingers)postoperatively. There was a 5.0 mm ×5.0 mm area necrosis of the flaps in 1 case (1 finger), which were cured after dressing treatment. 15 cases (19 fingers) were fellowed up for 3 to 12 months. And the flap had the same skin color as the finger. The two-point discrimination was 8 to 10 mm, averaged 9 mm. The activity of the interphalangeal joint was normal except the injured joint. Conclusion In repairing of finger wound, flap pedicled with dorsal cutaneous branch perforator of digital artery rebuild finger exterior by simpler surgery procedures, and have certain clinical application value.%目的：探讨指动脉终末背侧穿支血管链皮瓣修复指端缺损的疗效。方法2013年6月-2015年6月，对20例25指指端缺损创面采用同指指动脉终末背侧穿支血管链逆行岛状皮瓣进行修复。结果术后19例24指皮瓣全部成活，1例皮瓣远端约5.0 mm×5.0 mm创面发黑坏死，经换药后创面延期愈合。15例19指获随访3～12个月，皮瓣及植皮与患指颜色基本相同，皮瓣两点辨别觉为8～10 mm，平均为9 mm。除受损指关节外，余关节活动自如，无功能障碍。结论利用指动脉终末背侧穿支血管链皮瓣修复指端缺损操作简便，受区相似重建好，具有一定的临床应用价值。
