Objective To investigate symptoms in surgically treated colorectal cancer patients undergoing chemotherapy,and to identify symptom clusters.Methods A total of 110 surgically treated colorectal cancer patients undergoing chemotherapy were surveyed using a general information questionnaire and the M.D.Anderson Symptom Inventory-Gastrointestinal Cancer Module (MDASI-GI).Results These patients experienced multiple symptoms,including distress,poor appetite,sadness,fatigue,nausea,sleep dis turbance and drowsiness,etc.Spearman correlation analysis showed that several symptoms were positively correlated with each other,and the top five ranked correlation coefficients were,in order,coefficient between sadness and distress,nausea and poor ap petite,nausea and fatigue,poor appetite and fatigue,poor appetite and sleep disturbance.Principal component analysis with a varimax rotation,combining the scree plot and cluster analysis,revealed 5 symptom clusters:digestive-psychological,disease-related,vomiting-numbness,diarrhea-oral related,pain-abdominal distention,which explained 61.479％ of the total variance.The Cronbach α coefficients were 0.874,0.797,0.756,0.802 and 0.868 respectively.Conclusion Surgically treated colorectal cancer patients had multiple symptom clusters during chemotherapy,indicating nurses should assess their symptoms and take targeted measures for symptom management.%目的 了解结直肠癌患者术后化疗期间的相关症状,并探讨症状群的种类和数量.方法 采用一般资料问卷、安德森症状量表-胃肠道肿瘤模块中文版(MDASI-GI)对110例术后化疗结直肠癌患者进行调查.结果 结直肠癌患者术后化疗期间存在多种症状,主要有苦恼、食欲差、悲伤、疲劳、恶心、睡眠不安和瞌睡等;Spearman相关分析显示结直肠癌患者术后化疗期间的多个症状间呈正相关,其中相关系数居前5位从高到低依次为悲伤与苦恼、恶心与食欲差、恶心与疲劳、食欲差与疲劳、食欲差与睡眠不安;主成分分析结合方差最大正交旋转法,再综合碎石图和聚类分析得出5个症状群,分别为消化道-心理症状群、病感症状群、呕吐-麻木症状群、腹泻-口腔相关症状群、疼痛-腹胀症状群,Cronbach's α系数分别为0.874、0.797、0.756、0.802和0.868,累积方差贡献率为61.479％.结论 结直肠癌患者术后化疗期间存在多个症状群,护理人员应及时做好症状群评估,并采取针对性措施进行症状群管理.
