经兔耳背动脉留置针反复采血后局部血管组织炎性细胞因子表达的实验研究

         

摘要

目的:观察经兔耳背动脉留置针不同频率采血,局部血管损伤炎性细胞因子的变化,为临床动脉留置针的科学规范使用及动脉血管的保护提供动物实验依据。方法将新西兰大白兔48只按随机数字表法随机分为对照组(n=12)、实验组 A 组(n=12)、实验 B 组(n=12)、实验 C 组(n=12)。对照组经兔耳背动脉留置针使用浓度为125 U/mL 肝素液2 mL 封管,2次/d。实验组经兔耳背动脉留置针不同频率采集血标本,实验 A 组共采血64次,实验 B 组共采血32次,实验 C 组共采血16次,每次采血完毕均用浓度为125 U/mL 肝素液2 mL 封管。96 h 后,对48只大白兔于穿刺点前2 cm 处取动脉血管和局部组织的标本,切片行免疫组化,检测肿瘤坏死因子-α、白细胞介素-1β、白细胞介素-10的表达。结果肿瘤坏死因子-α、白细胞介素-1β、白细胞介素-10在实验A组、实验 B 组、实验 C 组、对照组的表达均依次减弱,分别比较各组间肿瘤坏死因子-α、白细胞介素-1β、白细胞介素-10表达的积分光密度值,差异有统计学意义(P 均<0.05)。结论兔耳背动脉留置针保留96 h,经留置针处采血频率越高致肿瘤坏死因子-α、白细胞介素-1β、白细胞介素-10表达越明显,说明动脉血管损伤炎症反应越严重。%Objective To observe the change of inflammatory cytokines in local arterial injury induced by different frequencies of blood sampling and to provide reference for the standardized usage of arterial indwelling needle and the protection of artery. Methods Forty-eight New Zealand white rabbits were randomly divided into control group, experiment group A, experiment group B and experiment group C, with 12 in each group. Different times of blood sampling were performed in experiment groups while in control group, arterial indwelling needles were locked by 2 mL heparin solution with a concentration of 125 U/mL, with 2 times per day. Artery and local tissue 2 cm in radius for puncture point after 96 h were taken for immunohistochemistry to determine the expression of tumor necrosis factor α, interleukin1β and interleukin10. Results The expression of tumor necrosis factor α, interleukin1β and interleukin10 reduced in experiment group A, experiment group B and experiment group C and control group in sequence. The difference of integrated optical density of the expression of tumor necrosis factor α; interleukin1β and interleukin10 in each group indicated statistical significance (all P <0.05). Conclusion The significant expression of tumor necrosis factor α, interleukin1β and interleukin10 in arterial indwelling indicates serious inflammatory reaction in local arterial injury when arterial indwelling needle remains 96 hours.
