四川省2931例社区老年高血压糖尿病患者护理服务的满意度及影响因素分析

         

摘要

Objective To investigate the satisfaction with nursing care of chronic illness management of elderly patients with hypertension and diabetes,and analyze its influencing factors. Methods With convenient sampling, 2,931 elderly hypertensive patients and diabetic patients in 19 community health service centers of Sichuan province were investigated using the patient satisfaction questionnaire with nursing care of chronic illness management. Results The general satisfaction rate was 78.38%. Sub-domains with lower satisfaction rates were health education and guidance (73.62%), nursing follow-up (74.11%) and collaborative care (74.40%);nursing interventions with lower satisfaction rates were psychological counseling (71.20%), rehabilitation guidance (73.19%), directing how to maintain a healthy lifestyle (73.21%). Multiple linear regression analysis revealed the influencing factors of patients ' satisfaction were the residence, special outpatient, out-of-pocket medical expenses, personal monthly income, family medical history and drinking (P<0.05). Conclusion Patients' satisfaction rate of nursing care of chronic illness management is not high, especially that of health education and guidance, follow-up nursing care and collaborative care. To improve quality of nursing care related chronic illness case management, it is urgent to strengthen special training about community health nursing and chronic illness for nurses.%目的 调查四川省社区老年高血压、糖尿病患者护理服务的满意度,并分析其影响因素,为改进慢病管理的护理服务提供依据.方法 采用便利抽样,应用美国中华医学基金会项目组制定的社区慢性病管理服务满意度问卷,在四川省19家社区卫生服务机构对2931例老年高血压、糖尿病患者进行了调查.结果 本组老年患者对慢性病管理护理服务的总体满意度为78.38%,满意度较低的维度是健康教育和指导(73.62%)、护理随访(74.11%)和协同服务(74.40%),满意度较低的护理措施是心理疏导(71.20%)、康复指导(73.19%)和指导维持健康生活方式(73.21%).多元逐步回归分析结果显示患者所在区域、特殊门诊、自付费用、个人月收入、家族史、饮酒影响患者护理服务满意度(P<0.05).结论 四川省社区老年高血压、糖尿病患者护理服务满意率不高,突出表现在健康指导、护理随访和协同服务方面.要提升慢性病管理中护理服务的质量,迫切需要加强社区护士的专科知识和技能的培训.
