文章以健康教育为出发点,聚焦于慢性病,根据目前国内自我管理应用的状况,从家庭、社区、医院等方面综述了健康教育在慢性病患者自我管理中的应用,包括健康教育模式以及健康教育的实施方式,并对这些模式进行了讨论分析,指出健康教育中应注意的问题.%This paper mainly focused on health education and self - management of patients with chronic diseases. It reviewed the current situation of health education used in self - management of patients with chronic diseases from the aspects of family, community, hospital and so on, including health education modes and health education means. It analyzed these modes and pointed out the problems that should be paid attention to.
