Objective To explore the repairing effect of self-acid etching adhesion repairing system in the treatment of dentition defect accompanied by severe attrition in the elderly patients. Methods A total of 68 elderly patients with dentition defect accompanied by severe attrition who were admitted in our hospital from April, 2012 to October, 2015 were included in the study and randomized into the observation group and the control group with 34 cases in each group. The patients in the control group were given the total acid etching adhesion repairing system, while the patients in the observation group were given the self-acid etching adhesion repairing system. A follow-up visit of 12 months was paid after treatment. The therapeutic effect in the two groups was compared. The scores of mastication, gastrointestinal function, dentition appearance, pronunciation, and psychological state in the two groups were evalauted. Ryge was used to evaluate the sensitivity of dentin in the two groups. The periodontal index after treatment in the two groups was detected and recorded. Results No secondary decayed teeth occurred after treatment for 12 months, the color of affected tooth and obturator was not changed, and the dental pulp activity was normal. With the time extending, the scores of mastication, gastrointestinal function, dentition appearance, pronunciation, and psychological state at1 week, 1, 3, 6 and 12 months after treatment in both groups were gradually increasing;moreover, the various scores in the observation group were significantly higher than those in the control group (P＜0.05 or ＜0.01).The proportion of dentin sensitivity with different grades at 1 week, 1, 3, 6, and 12 months after treatment in the observation group was significantly lower than that in the control group (P＜0.05). The comparison of the proportion of dentin sensitivity 12 months after treatment between the two groups was not statistically significant (P＞0.05). The comparison of m SBI, PIS, PD, and m PLI after treatment between the two groups was not statistically significant (P＞0.05). Conclusion The self-acid etching adhesion repairing system in the treatment of dentition defect accompanied by severe attrition in the elderly patients can improve the mastication, gastrointestinal function, dentition appearance, pronunciation, and psychological state, and effectively repair and protect the dentin sensitivity.%目的 探讨自酸蚀粘接修复系统对老年牙列缺损伴重度磨耗的修复效果.方法 选取2015年4月-2016年10月我院收治的老年牙列缺损伴重度磨耗患者68例,共98颗缺损牙,随机分为观察组(47颗)和对照组(51颗),每组34例;对照组采用全酸蚀粘接修复系统治疗,观察组采用自酸蚀粘接修复系统治疗,治疗后随访12个月.比较两组治疗效果;分别对两组患者治疗后咀嚼、胃肠功能、牙列外观、发音、精神心理状态等指标进行评分,采用Ryge评价两组患者牙本质敏感性;检测并记录两组治疗后牙周指数.结果 治疗后12个月内两组均无继发龋齿,患牙的牙体与充填体均无色变,牙髓活力正常;于治疗后1周、1个月、3个月、6个月、12个月观察组和对照组咀嚼、胃肠功能、牙列外观、发音、精神心理状态各项功能评分随时间的延长呈逐渐上升趋势,且观察组各项评分均高于对照组(P＜0.05或＜0.01).治疗后1周、1个月、3个月、6个月观察组不同等级牙本质敏感性患者比例均低于对照组(P＜0.05),治疗后12个月两组患者牙本质敏感性患者比例差异无统计学意义(P＞0.05).治疗后两组患者龈沟出血指数(m SBI)、牙龈乳头指数(PIS)、牙周探诊深度(PD)、改良菌斑指数(mPLI)差异均无统计学意义(P均＞0.05).结论 自酸蚀粘接修复系统可改善老年牙列缺损伴重度磨耗患者的咀嚼、胃肠功能、牙列外观、发音、精神心理等各种功能,对牙本质敏感性具有有效的修复和保护作用.
