下尿路感染患者尿液病原菌分布及其耐药性分析

Objective To analyze pathogen composition and distribution and antibiotic resistance in the patients with lower u -rinary tract infection ( LUTI) , so as to provide evidence for reasonable application of antibiotics clinically .Methods Urine samples of 138 patients with LUTI were collected from Jan .2013 to Dec.2013.Then, the identification of pathogens and sensitive tests of antibi-otics were performed in the study .Results A total of 135 strains was isolated, including 93 strains of Gram-negative (68.89%), 40 strains of Gram-positive (29.63%) and 2 strains of fungi (1.48%).Results indicated that the most commonly-seen pathogens were 48 strains of Escherichia coli (35.56%), 21 strains of Klebsiella pneumonia (15.56%), 20 strains of Enterococcus (14.81%), 18 strains of Staphylococcus (13.33%) and 15 strains of Proteus mirabilis (11.11%).Main Gram-negative strains were 100%susceptible to imi-penem, and piperacillin/tazobactam, cefoperazone/sulbactam and amikacin showed relatively high sensitivity .Main Gram-positive strains were nearly 100%susceptible to linezolid and vancomycin and also susceptible to vancomycin , with the exception that there was only 1 case of Enterococcus strain with drug resistance.Besides, Gram-positive strains were also highly susceptible to nitrofurantoin .Conclusion The pathogens in the LUTI patients were generally resistant to most commonly-used antibiotics .Clinical physicians should intensify mo-nitoring of drug resistant strains , and antibiotics should be carefully chosen in accordance with the testing results of drug resistance .%目的 分析下尿路感染(lower urinary tract infection,LUTI)患者尿液中的病原菌分布、构成及耐药情况,为临床抗菌药的合理使用提供参考.方法 采集武警总医院2013年1-12月138例LUTI患者的尿液标本,进行细菌种类鉴定和药敏试验.结果 共分离到病原菌135株,其中革兰阴性菌93株(68.89%)、革兰阳性菌40株(29.63%)、真菌2株(1.48%).最常见的病原菌依次为大肠埃希菌48株(35.56%)、肺炎克雷伯菌21株(15.56%)、肠球菌属20株(14.81%)、葡萄球菌属18株(13.33%)、奇异变形杆菌15株(11.11%).主要革兰阴性菌对亚胺培南100%敏感,对哌拉西林/他唑巴坦、头孢哌酮/舒巴坦、阿米卡星的敏感率较高.主要革兰阳性菌对利奈唑胺100%敏感,对万古霉素敏感率高(仅肠球菌属发现1例耐药菌株),对呋喃妥因的敏感率也较高.结论 LUTI病原菌对大多数常用抗菌药均产生了不同程度的耐药性,临床应加强对耐药菌的监测,并根据药敏试验结果合理选用抗菌药.
