术前逼尿肌功能状态对良性前列腺增生患者PKRP术后膀胱痉挛的影响

摘要

目的 探讨术前逼尿肌功能状态对良性前列腺增生(BPH)患者经尿道双极等离子前列腺电切术(PKRP)后膀胱痉挛的影响.方法 选取2016年8月至2017年9月期间本院泌尿外科收治的BPH患者103例为研究对象,根据术前逼尿肌是否稳定分为逼尿肌稳定(DS)组和逼尿肌不稳定(DI)组.比较两组手术一般情况、两组术后3d膀胱痉挛发生情况、术后1～3 d膀胱痉挛次数、痉挛持续时间以及疼痛程度.结果 两组手术时间、膀胱冲洗时间以及留置尿管时间比较,差异无统计学意义(P＞0.05);DS组术后3d内膀胱痉挛发生率为48.28％,显著低于DI组的71.11％ (P＜0.05);术后1d、2d、3d,DS组痉挛次数、持续时间、视觉模拟评分法(VAS)评分均显著优于DI组(P＜0.05).结论术前逼尿肌功能状态与BPH患者PKRP术后膀胱痉挛有一定关系,术前逼尿肌不稳定患者更易发生膀胱痉挛,且膀胱痉挛程度较为严重.%Objective To explore the effects of preoperative detrusor functional status on bladder spasm in patients with benign prostatic hyperplasia (BPH) after plasma kinetic resection of the prostate (PKRP).Methods A total of 103 BPH patients admitted to the Department of Urology in our hospital during Aug.2016 and Sept.2017 were selected and divided into the detrusor stability group (DS group) and detrusor instability group (DI group) according to the functional status of detrusor before operation.The general information,incidence of bladder spasm 3 d after operation,times,duration and pain degree of bladder spasm 1-3 d after operation of the two groups were compared.Results There were no significant differences in the operation time,bladder irrigation time and catheter indwelling time between the two groups (P ＞ 0.05).The incidence of bladder spasm in the DS group was significantly lower than that in the DI group (48.28 ％ vs.71.11％) (P＜0.05).The times,duration and visual analogue scale (VAS) score of bladder spasm in the DS group were significantly superior to those in the DI group on day 1,2 and 3 after operation (P＜0.05).Conclusion Preoperative detrusor functional status has a certain relationship with bladder spasm in BPH patients after PKRP surgery,and patients with detrusor instability are more likely to have bladder spasm and the spasm tends to be more serious.
