欧洲五维健康量表在三线城市居民健康评价中的应用研究

摘要

Objective To conduct the application research on health utility in the residents of Wuhu as a representative third-tier city by using the European quality of life 5-dimensions(EQ-5D) for analyzing the relationships between different time trade-off(TTO) conversion model and other factors,and comparing the goodness of fit between the Japan(JP) TTO conversion model with the United Kingdom(UK) TTO conversion model. Methods By using the questionnaire survey method,EQ-5D was adopted to measure the survival quality in the residents of Wuhu as a representative third-tier city. Then the TTO conversion mod-els of Japan and UK were utilized to calculate the health index of the research subjects. The correlation between the demographic factors with the visual analogue scales(EQ-VAS) score was analyzed by using the SPSS19.0 software. The index for testing the goodness of fit model was calculated as well. Results 782 citizens were conducted the questionnaire survey ,the ratio of male to female basically reached 1∶1. By using different TTO models to calculate the health index score ,the score of Japanese TTO model was higher than that of the UK TTO model,moreover these two kinds of TTO conversion model had certain correlation with the age,cultural level and EQ-VAS. Calculating the values of R2,F,AIC and BIC and conducting comparison found that the goodness of fit of JP model was better than that of UK model. Conclusion Different EQ-5D TTO transform models have slight difference in the health indexes measurement of residents in Wuhu as a representative third-tier city. Comparing the indexes for testing the goodness of fit for the two kinds of model finds that the JP model is more suitable for the study population than the UK model.%目的：采用欧洲五维健康量表（EQ-5D），在代表性三线城市芜湖对居民的健康效用进行应用性研究。分析不同时间权衡量法（TTO）转换模型与其他因素之间的关系，并比较英国TTO转换模型与日本TTO转换模型在拟合该研究人群时的拟合优度。方法通过问卷调查的方法，用EQ-5D测量以芜湖为代表的三线城市居民的生存质量，利用日本和英国的TTO转换模型计算研究对象的健康指数。应用SPSS19.0统计软件分别分析2种模型与人口学因素及视觉模拟标尺得分（EQ-VAS）的相关性，并计算用于检验模型拟合优度的指标。结果对782名市民进行了问卷调查，男女比例基本达到1∶1。应用不同TTO模型对健康指数得分进行计算，可以看出日本模型的分数高于英国模型，且这2种TTO转换模型与年龄、文化程度及EQ-VAS之间具有一定的相关性。通过计算R2值、F值、赤池信息准则值及贝叶斯系数值并进行比较发现，与英国模型比较，日本模型的拟合优度更好。结论不同EQ-5D的TTO转换模型在测量以芜湖为代表的三线城市居民健康指数中略有不同。通过比较用于检验2种模型拟合优度的指标发现，日本模型较英国模型更适用于研究人群。
