目的：探讨采用支气管肺泡灌洗液治疗非特异性间质性肺炎的临床疗效。方法：选取了2011年11月1日－2014年5月1日我院门诊收治的非特异性间质性肺炎（IPF ）患者共25例，设为观察组（IPF组）；同时选取隐源性机化性肺炎（COP）20例，设为对照组（COP组）。观察两组患者灌洗液中不同细胞组分与肺功能指标之间的关系，对各组细胞分类以及生存率的差异进行研究分析。结果：对比结果显示，两组患者在肺泡巨噬细胞及淋巴细胞比例上差异显著，COP组淋巴细胞比例较高，中性粒细胞比例的差别无统计学意义，但IPF组中性粒细胞比例与COP组相比偏高，其差别有统计学差异。预后情况中，研究发现淋巴细胞比例较高的IPF患者预后较好，淋巴细胞比例增加易导致死亡风险的上升。结论：不同类型的间质性肺炎其支气管肺泡灌洗液细胞类型也具有显著差异，淋巴细胞比例升高为疾病严重程度及预后的保护性因素，中性粒细胞比例升高可能为影响预后的不良因素。%Objective :To evaluate the clinical efficacy of the use of bronchoalveolar lavage fluid treatment of non‐spe‐cific interstitial pneumonia .Methods :Select the November 1 ,2011 to May 1 ,2014 admitted to our hospital nonspecific interstitial pneumonia (IPF) patients with a total of 25 cases ,set the observation group ;meanwhile selected 20 cases of cryptogenic organizing pneumonia (COP) ,set in the control group .Observe the relationship between the different cel‐lular components and lung function between the two groups of patients lavage fluid ,as well as differences in the classi‐fication of cells in each group survival analysis .Results :Comparison showed that both groups of patients in the alveolar macrophages and lymphocytes were significantly different ,higher COP group proportion of lymphocytes ,neutrophils percentage difference was not statistically significant ,but the proportion of neutrophils IPF group compared with the COP is high ,the difference is statistically significant .In prognosis ,the researchers found that a higher proportion of lymphocytes in patients with a better prognosis of IPF ,increase the proportion of lymphocytes easily lead to increase in the risk of death .Conclusion:Different types of interstitial pneumonia in bronchoalveolar lavage fluid cell types which also have significant differences in the proportion of lymphocytes increased as disease severity and prognosis of protec‐tive factors ,the proportion of neutrophils may increase adverse prognostic factors .
