关节炎患者免疫球蛋白和血小板参数与病情活动的类风湿性相关性探讨

摘要

Objective:To investigate the relationship between immunoglobulin and platelet parameters in the patients with rheumatoid arthritis and disease activity .Methods :76 patients with RA in our hospital were used as the study group .40 cases were active RA ,the other 36 cases were remission of RA ,selected 40 healthy persons as control group at the same period .Measured serum markers AKA and anti‐CCP antibodies in patients with RA and healthy people . Then calculated the detection rate in the two groups ;Measured the immunoglobulin and platelet parameters in the RA patients and healthy people ,compared their expression activity in different conditions and the relationship with AKA and anti‐CCP antibodies .Results:The positive rate in the active RA and remission of RA were significantly higher than the control group which was measured by AKA and anti‐CCP antibodies ,and the positive rate was higher in active ,the difference was statistically significant (P<0 .05);the immunoglobulin and platelet parameters were significantly higher in RA patients ,the difference was statistically significant (P<0 .05);the serum immunoglobulin IgA ,IgM and platelet parameters PLT and the PCT have a positive linear correlation with AKA and anti‐CCP antibodies .Conclusion:The ser‐um immunoglobulin and platelet parameters in RA patients at the early detection of disease Can reflect the disease ac‐tivity timely which can provide reference for the early diagnosis and treatment .%目的：探讨类风湿性关节炎（RA ）患者免疫球蛋白和血小板参数与病情活动的相关性。方法：选取我院2012年5月－2015年1月在我院初诊为RA的76例患者作为研究组，40例为活动期RA ，36例为临床缓解期RA ，另选取同期在院体检的40例健康者作为对照组。分别测定RA患者及健康者血清标志物AKA及抗CCP抗体，计算其在两组研究者体内的阳性检测率；分别测定RA患者及健康者免疫球蛋白和血小板参数，比较其在不同病情活动者中的表达情况，并分析其与AKA和抗CCP抗体的相关性。结果：AKA和抗CCP抗体检测活动期RA及缓解期RA阳性率均明显高于对照组，且活动期RA阳性率高于缓解期RA阳性率，差异有统计学意义（P＜0．05）；RA患者免疫球蛋白及血小板参数表达水平均明显高于对照组，且在活动期RA中的表达量明显高于缓解期RA ，差异有统计学意义（ P＜0.05）；血清免疫球蛋白IgA、IgM及血小板参数PLT及PCT与AKA及抗CCP抗体呈直线正相关。结论：RA患者的血清免疫球蛋白及血小板参数的早期检测，可及时反映其病情活动变化，为临床早期诊断和治疗提供参考依据。
