褪黑素对重症胰腺炎早期预测指标胰蛋白酶原激活肽及血清淀粉样蛋白A的影响

     

摘要

目的 探讨褪黑素对重症急性胰腺炎早期预测指标胰蛋白酶原激活肽(TAP)及血清淀粉样蛋白A(SAA)的影响.方法 54只SD大鼠随机分为假手术组(SO组)、重症胰腺炎组(SAP组)、MT干预组(MT组),每组18只.应用胰胆管逆行注射牛磺胆酸钠的方法诱导SAP模型,MT组于诱导模型前30min腹腔注射MT.术后4、8和12h分批处死大鼠(每个时间点n=6),ELISA法检测血清MT及TAP浓度,免疫比浊法测定血清SAA水平,同时检测血清淀粉酶,胰腺组织行病理检查.结果 SAP组胰腺病理损伤,MT组胰腺病理损伤较SAP组明显减轻(P<0.05);SAP组淀粉酶较SO组明显增高(P<0.05或P<0.01),而MT组较相应时间点SAP组显著降低(P<0.05);血清MT浓度在SAP组随病情进展逐渐降低,MT组较相应时间点SAP组增高(P<0.05);与SO组相比,SAA、TAP水平在ANP组明显增高(P<0.01),而MT组较相应时间点SAP组显著降低(P<0.05).结论 褪黑素干预能降低SAP早期预测指标TAP及SAA浓度,对胰腺炎起保护作用.%Objective To study the influence of melatonin on trypsinogen activation peptides(TAP) and serum amyloid A(SAA) in early assessment of severe acute pancreatitis. Methods Fifty -four male Sprague - Dawley (SD) rats were randomly divided into three groups; sham - operation group, SAP group and MT - pretreated group. The models of SAP were induced by retrograde injection sodium taurocholate into the bili - pancreatic duct. MT group undergoing intraperitoneal injection 50 mg/kg 30 minutes before the establishment of SAP models. 4, 8 and 12 hours after the onset of operation, the concentrations of serum MT and TAP were analyzed by ELISA method. Serum amyloid A levels were measured by Latex Immunoturbidometry, and serum amylase and pathological changes of the pancreas were observed. Results Pancreatic pathological damage in SAP groups was progressive exacerbated. It was obviously ameliorated in MT group as compared with SAP group (P <0. 05). Compared with SO group, the levels of serum amylase were significantly increased in SAP group (P < 0. 05 or P < 0. 01). They were markedly decreased in MT group as compared with SAP group ( P < 0. 05). In SAP group, MT leveles were decreased with the disease progress. MT group had a higher concentration of MT than that of SAP group. Compared with SO group, the levels of SAA,TAP were significantly increased in SAP group {P <0. 05or P <0. 01). They were markedly decreased in MT group as compared with SAP group (P < 0. 05). Conclusion Melatonin can reduce the TAP and SAA levels in early stage of severe acute pancreatitis.
