Objective Nowadays it is focused on wound care with miner or no pain. Some studies indicated that local anesthetic infiltrations and atraumatic dressings or moisture therapy can diminish pain. Trolamine cream was used to prevent and treat radiation dermatitis at first and now it is used in wound treatment that can control inflammation and provide a moisture environment. However, it is unknown whether it can reduce wound pain and enhance wound healing. Our study aims to evaluate the effects and safety of traumatic wounds care with trolamine cream for miner or no pain. Methods A total of 61 patients were randomly divided into trial (31) and control (30) groups. Baseline data were comparable between the two groups. Trial group wounds were smeared with trolamine cream plus vaseline and gauze covering. Control group wounds were covered with alginate dressing as standardized moisture therapy. Observed parameters; wound pain scores when dressings were changing and after treatment at day 3,7, 14 and 21; secondary traumatic scores; compliance scores; healing rate; healing time and dressing changed times. All the data were analyzed by an SPSS 11.0 statistic soft ware. Results One case in trial group was excluded because he accepted less than one week of treatment but failed the follow-up. There were 30 cases in each group completed the wound care. The healing rate was 100% for both groups. The mean healing time of trial group was shorter than that of control group significantly (P <0. 05). The mean dressing changing times of trial group were less than those of control group significantly (P <0. 01) . The differences of mean secondary trauma scores and compliance scores between the two groups were not significant (P >0. 05) . The wound pain scores dropped at treatment day 3 , but there was no significant difference between the two groups (P > 0. 05). The scores of wound pain at day 7 and 21 and dressing changing pain at day 3, 7 and 14 for the trial group were lower than those for the control group ( P < 0. 05 ). Conclusion Trolamine cream can reduce traumatic wound pain with short healing time and less dressing changing times. The effect of trolamine cream is better than that of alginate dressing. It is safe and easy to use.%目的 三乙醇胺乳膏是用于防治放射性皮炎的有效复合乳剂,新近发现能消炎和保持伤口湿润,可用于伤口治疗,但对伤口减痛促愈的效果评价尚缺乏研究报道.文中旨在探讨三乙醇胺乳膏对创伤性伤口的减痛促愈效果和使用安全性. 方法 入选61例创伤性伤口患者,随机分为观察组和对照组,分别为31例和30例,基线处理结果显示2组有可比性.观察组采用三乙醇胺乳膏涂抹加保湿覆盖,对照组采用藻酸盐敷料覆盖.观察指标:治疗后3、7、14、21 d观察伤口疼痛计分、揭敷料时疼痛计分、敷料2次创伤积分、患者依从性计分、治愈率和愈合时间及敷料更换次数. 结果 观察组1例患者因治疗不足1周失访而剔除.观察组和对照组各有30例患者完成治疗,2组治愈率100％,观察组伤口愈合时间短于对照组(P＜0.05),敷料更换次数少于对照组(P＜0.01),2组患者2次创伤积分和依从性计分无统计学意义(P＞0.05).减痛效果:伤口处理3d后2组患者疼痛均有减轻,但差异无统计学意义(P＞0.05),7d和21d时,观察组患者疼痛计分较对照组明显减少(P＜0.05).3、7和14d揭敷料时的疼痛,观察组明显轻于对照组(P＜0.05). 结论 三乙醇胺乳膏用于创伤性伤口,其减轻伤口疼痛和揭敷料时的疼痛、缩短愈合时间和减少敷料更换次数的效果优于单用藻酸盐敷料,使用安全便捷.
