多层螺旋CT血管造影在右肺上叶动脉解剖中的应用

         

摘要

Objective To study the pulmonary arteries of the right upper lobe with multi-slice spiral CT angiography combined with the post-processing technology of volume rendering .Methods Totally 124 patients underwent multi-slice CT pulmonary ar-tery angiography .Display the right upper artery with volume rendering technology , then classify the right upper pulmonary arteries and study the segmental arteries by the volume rending technology .Results 1) Right upper pulmonary arteries can be classified into seven types:“anterior trunk+posterior ascending artery”type (54.0%),“anterior trunk+anterior ascending artery +poste-rior ascending artery”type ( 12.9%), “superior anterior trunk +inferior anterior trunk +posterior ascending artery” type (12.1%),“anterior trunk+ascending artery” type (8.1%),“anterior trunk”type (7.3%),“superior anterior trunk +inferior anterior trunk” type (4.0%),“anterior trunk+anterior ascending artery” type (1.6%).Right upper pulmonary arteries .The type of two branches type is the majority in the right upper pulmonary arteries ( 68%) .Then is the the type of three-branches (25.0%).The least is the type of one branch (7%);2) The sources of the segmental pulmonary arteries of the right upper lobe are relatively complex .But one branch which is more than half in all is in the majority .Conclusion The right upper lobepulmo-nary artery can be displayed visually displayed by multi-slice spiral CT angiography combined with the volume rendering technolo-gy, which helps to instruct the thoracic surgery to consume its security and accuracy .%目的:应用多层螺旋CT血管造影( MSCTA)联合后处理容积重建( VR)技术显示右肺上叶动脉,对其解剖结构进行分类。方法124例患者行MSCT肺动脉造影,应用VR技术显示右肺上叶动脉,研究右肺上叶动脉的分支类型及各段动脉的来源。结果1)右肺上叶动脉有7种分支类型:“前干+后升动脉”型(54.0%)、“前干+前升动脉+后升动脉”型(12.9%)、“上前干+下前干+后升动脉”型(12.1%)、“前干+升动脉”型(8.1%)、“前干”型(7.3%)、“上前干+下前干”型(4.0%)、“前干+前升动脉”型(1.6%)。总结以上分支类型,其中以二支型最多见(67.7%),其次为三支型(25.0%),一支型最少(7.3%);2)右肺上叶各段动脉来源较为复杂,但均以一支动脉来源居多,且都超过半数。结论MSCTA联合后处理VR技术可以真实直观显示右肺上叶动脉的分支形式并对其准确分型,可以指导胸外科手术。
