急诊气管插管患者首次口腔护理的不同时间比较分析

摘要

目的：：探讨急诊气管插管患者首次口腔护理的适宜时间。方法：随机选取某院收治的急诊行气管插管患者203例,将插管后无需急诊手术可立即口腔护理的患者定为对照组(51例),其余随机分为3组,即 I 组(49例)、II 组(53例)、III 组(50例),分别于插管时、插管后第6h、8h、10h 进行首次口腔护理,护理后的治疗和干预均一致。比较各组在首次口腔护理后2h 和4h 的牙菌斑评分和培养结果差异。结果：口腔护理后2h,各组的牙菌斑评分具有显著差异(P <0.01),II 组的牙菌斑评分显著低于其他组(P <0.01),护理后4h 的牙菌斑评分不存在显著差异(P >0.05)；4组患者口腔护理后2h 的细菌培养结果差异具有显著性(P <0.01)；III组的平均值显著高于其他组(P <0.01)；对照组、I、II 组的口腔护理后4h 结果无显著差异(P >0.05)。结论：插管后第8h 行口腔护理其抑制牙菌斑果优于第6、10h。抑制细菌量则6、8h 优于第10h,综合考虑以插管后第8h 进行口腔护理较适宜。%Objective:To explore the appropriate time for the first oral care of intubated patients in emer-gency first oral care.Methods:Randomly selected 203 cases of intubated patients in emergency in our hospi-tal.The patients who would not need emergent surgery after intubation and could immediately receive oral care served as the control group (5 1 cases),and the rest were randomly divided into three groups,namely, Group I (49 cases),II group (53 cases),III group (50 cases),respectively.All of the patients received oral care at the time of intubation and after the intubation 6h,8h,10h respectively.The treatment and interven-tions after care are consistent.Compare plaque scores and culture results in each group after the first 2h and 4h of oral care.Results:After 2h of oral care,the plaque scores of the two groups are significantly different (P <0.01 ).Plaque score of group II was significantly lower than that of other groups (P <0.01 ).After 4 hours of oral care,plaque score has no significant difference (P >0.05);2 hours after care,bacterial culture results of 4 groups were significant (P <0.01);the average of group III was significantly higher than other groups (P <0.01);after 4h of care,the control group I,II have significant difference (P >0.05).Conclu-sion:implementing oral care for the 8th hour after intubation,the effect of inhibiting plaque is better than that of the 6th hour and 10th hour.The quantity of inhibiting bacteria of the 6the hour and 8th hour is better than that of the 10th hour.In conclusion,oral care is relatively suitable implemented for the 8th hour after intubation.
