腹腔镜联合胆道镜治疗胆囊息肉合并胆囊结石患者的临床效果

摘要

Objective:To study the clinical effect of laparoscopy combined with choledochoscopy in treatment of cholecystolithiasis complicated with cholecystolithiasis.Methods:106 cases with gallbladder polyps with gallbladder stones in a hospital from September 2014 to August 2015 were randomly divided into two groups according to the computer, the laparotomy group using conventional open cholecystectomy surgery;treatment group were treated with endoscopy combined with laparoscopy.Then compare the whole operation time, blood loss during surgery in the two groups in total, the average recovery time after surgery and surgical treatment of gastrointestinal function, time to get out of bed activities of the total efficiency, postoperative bile leakage and incision infection and other complications.Results:The total effective rate of the combined operation in the treatment group was significantly higher than that in the laparotomy group.The incidence of postoperative complications such as bile leakage and incision infection was significantly lower than that of the open group, P<0.05.The operation time, the total amount of intraoperative blood loss, postoperative recovery of gastrointestinal function time, and the average time of getting out of bed were significantly better than those of the open group, P<0.05.Conclusion:The clinical effect of patients with laparoscopy combined with choledochoscopy in treatment of cholecystolithiasis and gallbladder polyps is effective, which can effectively improve the clinical symptoms, small trauma, simple operation, less bleeding, fewer complications, and accelerate the recovery of gastrointestinal function after the operation, early ambulation, worthy of promotion.%目的:研究腹腔镜联合胆道镜治疗胆囊息肉合并胆囊结石患者的临床效果.方法:以某院2014年9月~2015年8月胆囊息肉合并胆囊结石患者106例根据计算机随机法分两组,开腹组采用常规开腹切除胆囊手术治疗;双镜联合组采用腹腔镜联合胆道镜治疗.就两组患者手术操作全程时间、术中失血的总量、术后恢复胃肠道功能时间、开始下床活动的平均时间和手术治疗总有效率、手术后胆漏、切口感染等并发症发生率进行比较.结果:双镜联合组手术治疗总有效率明显高于开腹组,手术后胆漏、切口感染等并发症发生率明显低于开腹组,P<0.05.双镜联合组患者手术操作全程时间、术中失血的总量、术后恢复胃肠道功能时间、开始下床活动的平均时间均明显优于开腹组,P<0.05.结论:腹腔镜联合胆道镜治疗胆囊息肉合并胆囊结石患者的临床效果确切,可有效改善患者临床症状,创伤小,操作简单,并发症少,可加速术后胃肠功能恢复,值得推广.
