超声显示钙化灶对甲状腺良恶性结节的诊断意义

摘要

Objective To explore the diagnostic value of benign and malignant thyroid nodules of ultrasonic display.Methods The pathological examination results of 248 cases of thyroid operation in patients with preoperative thyroid color Doppler ultrasound images were retrospective studied.Results Display the pathological examination after operation of malignant thyroid nodules,there were 238 benign nodules.In the malignant group composed of calcification and microcalcification than were significantly higher than those in benign group(P<0.05);30 microcalcification, malignant thyroid nodules in 27,of which 15 for micro carcinoma, 12 papillary carcinoma. Calcification of small carcinoma and papillary carcinoma were 57.5%(23/40)and 66.7%(16/24),the micro calcification proportion was 37.5%(15/40)and 50%(12/24); malignant group 64 cases under 45 years old, 32 cases with calcification, accounted for 50%(32/64),micro calcification accounted for 37.5%(24/64);>45,7 cases had calcification, accounted for 10.9%(7/64),microcalcification accounted for 4.7%(3/64),compared the two groups had significant difference(P＜0.05).Conclusion Calcification of color Doppler ultrasound in the image is a specific marker for thyroid cancer,which can make the correct diagnosis of the majority of thyroid nodule, for clinical treatment to provide extremely important information.%目的 探讨超声显示钙化灶对甲状腺良恶性结节的诊断意义.方法 对248例行甲状腺手术患者的病理检查结果与其术前甲状腺彩色多普勒超声图像进行回顾性比较研究.结果 术后病理检查显示,甲状腺恶性结节64个,良性结节238个.恶性组中钙化灶及微钙化灶的构成比均显著高于良性组,差异有统计学意义(P＜0.05);30个微钙化灶中,恶性甲状腺结节27个,其中15个为微小癌,12个为乳头状癌.微小癌及乳头状癌的钙化灶构成比分别为57.5%(23/40)和66.7%(16/24),其微钙化灶构成比分别为37.5%(15/40)和50.0%(12/24);恶性组64例中,≤45岁的32例有钙化灶,占50.0%(32/64),微钙化灶占37.5%(24/64);＞45岁的7例有钙化灶,占10.9%(7/64),微钙化灶占4.7%(3/64),两组比较差异有统计学意义(P＜0.05).结论 彩色多普勒超声图像中的钙化灶是甲状腺癌的一项特异性指标,能对多数甲状腺结节做出正确的定性诊断,为临床治疗方案提供极为重要的信息.
