Objective To determine effect of combined electromagnetic irradiation and cognitive behavior therapy to the young adults with temporomandibular disorders (TMD). Methods One hundred seventy eight young adults with TMD were eligible for a randomized controlled clinical trial to clarify the research question from October 2009 to January 2011. The patients were allocated randomly with either electromagnetic irradiation alone or combined it with cognitive behavior therapy, with 89 patients in each group and followed up for half years. Tenderness of the TM joints and maximum opening between edges of the anterior teeth were measured to evaluate treatment effects. Results At 2, 4 weeks and half year after treatments the effective rates for control group and treatment group were 87. 64％, 89. 89％,65.17％, 82.02％ and 48.31％, 77.53％ respectively. Conclusion Effect of electromagnetic spectrum irradiation alone on TMD was limited. But the effect of combined the above therapy with cognitive be havior theory was superior with statistical significance.%目的 了解电磁波照射联合认知行为疗法治疗颞下颌关节紊乱病中咀嚼肌功能紊乱的效果.方法 收集咀嚼肌功能紊乱患者178例,按就诊顺序随机均分为2组,分别接受单纯电磁波照射治疗或电磁波照射联合认知治疗,随访半年,观察关节压痛及最大张口度等指标评价疗效.结果 单纯电磁波照射治疗组和电磁波照射联合认知治疗组接受治疗2周后,有效率为87.64%、89.89%(P>0.05);治疗4周后,有效率65.17%、82.02%(P<0.05);治疗半年后,有效率为48.31%、77.53%(P<0.01).结论 对颞下颌关节紊乱病中咀嚼肌功能紊乱类型的患者单纯通过电磁波进行躯体治疗效果有限,如联合认知行为疗法则可显著提高疗效.
