Objective To explore the superciliary arch - orbital lock hole approach the clinical effect of circulation aneurysms before surgery. MethodsA retrospective analysis in April 2012 - April 2015 in our hospital to accept the eyebrow bow - orbital lock hole into the way of surgical treatment of the clinical data of 40 patients with former circulation aneurysms, surgical method, the results of the analysis, to study the surgical indications and clinical curative effect.Results40 patients with only one operation is a success, 52 aneurysm clip, intraoperative clamp, clamp by mistake did not happen, postoperative intracranial angiography, parent artery in patients with unobstructed, surgical success rate of 100%. Postoperatively, 40 patients in only 1 case complicated with hydrocephalus, after treatment with bypass heal, incidence of complications was 2.5%. The Glasgow prognostic score, all patients was 92.5%, no heavy residue, deaths, see table 1. After six months follow-up period, 2 cases withintracranial infection, the recovery of targeted therapy is good, the rest of the patients life quality basic before return to aneurysm concurrent, no complications occurred.ConclusionThe superciliary arch - orbital lock hole into the road circulation aneurysms before surgery, high success rate, less complications, safety of minimally invasive, prognosis is good, worthy of clinical application.%目的：探讨经眉弓-眶上锁孔入路手术治疗前循环动脉瘤的临床效果。方法：回顾性分析2012年4月～2015年4月于我院接受经眉弓-眶上锁孔入路手术治疗的40例前循环动脉瘤患者的临床资料，阐述手术方法，分析治疗结果，研究手术适应症及其临床疗效。结果：40例患者均一次手术成功，52个动脉瘤均夹闭，术中未出现误夹、漏夹情况，术后行颅内血管造影检查，患者载瘤动脉通畅，手术成功率100%。术后，40例患者中仅1例并发脑积水，经分流术治疗后痊愈，并发症发病率2.5%。经格拉斯哥预后评分，全部患者治疗优良率92.5%，无重残、死亡病例，详见表1。术后半年随访期间，2例患者颅内感染，经针对性治疗恢复良好，其余患者生存质量基本恢复至动脉瘤并发前，无手术并发症发生。结论：经眉弓-眶上锁孔入路手术治疗前循环动脉瘤，成功率高、并发症少、安全微创，预后良好，值得临床应用。
