自拟养阴解毒汤治疗鼻咽癌放疗致急性放射性口腔黏膜炎的临床观察

摘要

目的 观察自拟养阴解毒汤治疗鼻咽癌放疗过程中所致急性放射性口腔黏膜的临床效果并探讨其机制.方法 将根治性放疗鼻咽非角化性鳞癌患者64例采用简单随机法分为治疗组与对照组各32例,治疗组于放疗前1周即开始服用养阴解毒汤;对照组以地塞米松加利多卡因加重组人粒细胞集落刺激因子加维生素B12混合溶液漱口,自放疗开始至放疗结束,期间两组进行疗效对照观察;并通过检测血清C反应蛋白(CRP)、转化生长因子-β1 (TGF-β1)及白细胞介素-6(IL-6)指标客观检测炎症反应情况.结果 治疗组急性放射性口腔黏膜炎的发病率明显低于对照组(P＜0.05);治疗组总有效率为96.88％,高于对照组的67.74％(P＜0.05);同时养阴解毒汤处理组血清CRP、TGF-β1及IL-6明显较对照组降低(P＜0.05).结论 养阴解毒汤防治放射性口腔黏膜炎效果良好.%Objective:To observe the clinical effect of Yangyin Jiedu Decoction on acute radiation oral mucosa caused by radiotherapy for nasopharyngeal carcinoma (NPC) and explore its mechanism.Methods:64 cases of esophageal malignancies were randomly divided into the treatment group and the control group,32 cases in each.The treatment group began taking Yangyin Jiedu Decoction 1 weeks before radiotherapy.The control group was treated with dexamethasone plus lidocaine and recombinant human granulocyte colony stimulating factor plus vitamin B12 mixed solution.From the beginning of radiotherapy to the end of radiotherapy,the two groups were compared with curative effect observation.The levels of serum CRP,TGF-β1 and IL-6 were used to detect the inflammatory response.Results:Incidence rates of acute radiation oral mucositis of the treatment were less than that of the control group (P＜ 0.05).The effect rate of the treatment group was 96.88％,while that of the control group was 67.74％(P＜ 0.05).The levels of serum CRP,TGF-β1 and IL-6 was significantly lower than the control group(P＜0.05).Conclusion:Yangyin Jiedu Decoction was effective on acute radiation oral mucositis caused.
