实时组织弹性成像在兔肝创伤中的应用价值

     

摘要

Objective To evaluate the application value of real-time tissue elastography( RTE ) in diagnosis of liver trauma in rabbits. Methods Thirty blunt traumatic lesions in different lobes of liver in 16 healthy New Zealand rabbits were established, RTE was used to measure the elasticity value of normal liver tissue and traumatic lesions by self-control methods 5 min, 30 min and 60 min after trauma, the tests were repeated three times and the values were averaged. The results were compared with those of pathology. Results Thirty hepatic trauma model was successfully established, the elasticity value of liver traumatic lesions tended to increase in the overall 5 min, 30 min and 60 min after trauma. There was significant difference of elasticity values of the three different time periods( P<0.05 ). General observation showed the local uplift of the surface of liver trauma lesion, hematoma formation, and microscopy showed liver capsule rupture, hemorrhage, and inflammatory cell infiltration in the surrounding liver tissue. Conclusion The elasticity of liver traumatic lesions tends to increase in the overall with the time. The elasticity values of liver trauma lesions are different from normal liver tissue, meanwhile which is valuable for diagnosis of liver trauma.%目的 探讨实时组织弹性成像在兔肝创伤中的应用价值.方法 16只健康新西兰兔分别选择不同肝叶制作钝性肝创伤模型30处.应用实时组织弹性成像技术测量正常肝组织和创伤灶创伤后5 min、30 min及60 min的弹性值,分析其变化趋势,并与病理结果作对照分析.结果 成功建立肝创伤灶30处.创伤后5 min、30 min及60 min肝创伤灶弹性值总体变化呈从低到高的趋势,3个时间点弹性值之间比较,差异有统计学意义(P<0.05).大体观察可见肝创伤灶表面局部隆起,形成血肿;镜下观察可见创伤部位肝包膜破裂,肝组织出血,周围肝组织炎细胞浸润.结论 不同时间点实验兔肝创伤灶弹性值变化呈逐渐增高的趋势,与正常肝组织弹性值不同,故实时组织弹性成像可能为临床诊断肝创伤提供一种新途径.
