全瓷全冠与高嵌体修复低矮磨牙的临床对比研究

     

摘要

目的:探讨用CAD/CAM设计制作玻璃陶瓷全冠和高嵌体修复低矮磨牙的临床疗效.方法:选择低矮磨牙(牙合龈距3~4 mm)行标准根管治疗后分成全冠修复组和高嵌体组,采用IPS e.max?CAD经西诺德CEREC AC (D3492)牙科修复计算机辅助设计制作系统制作全冠和高嵌体共185例,术后参照改良美国牙科协会评价标准(改良USPHS标准),从外形、边缘密合性、固位、牙龈健康、继发龋等方面进行评价并统计分析.结果:1年后随访显示两种修复方式差异无统计学意义;2年后结果显示全瓷高嵌体修复成功率较全瓷全冠高,具有统计学意义.结论:CEREC AC椅旁CAD/CAM系统制作全瓷全冠以及高嵌体对低矮磨牙进行修复均可取得良好效果,而全瓷高嵌体远期效果比全瓷全冠更为出色,可作为低矮磨牙修复的首选.%Objective:To compare the clinical effect of short molars restoring with full ceramic crown and onlay by the system of CEREC AC(D3492)CAD/CAM.Methods:One hundred and eighty-five short molars with height form crown to gingiva of 3~4 mm which meet the inclusion criteria were randomly divided into two groups,which were restorated by full ce-ramic crown and onlay by the system of CEREC AC(D3492)CAD/CAM.According to the USPHS Standard,clinical effect of restoration was evaluated statistically regarding the aspect of appearance,marginal adaptation,retention,gingival condition and secondary caries.Results:No statistically significance was found at the time point of 1-year follow-up between the full ceram-ic crown and onlay group for the clinical effect,while onlay group tended to be superior to full crown group with a statistically significance in success rate after 2 years.Conclusion:Ceramic full crown and onlay by the system of CEREC AC CAD/CAM had become the approaches which were widely used in clinic,moreover,onlay group showed a better advantage over crown group in restoration effect and could be served as a first choice for restoration of short molars.
