纤维支气管镜支气管肺泡灌洗术治疗重症肺部感染的临床观察

摘要

目的 探讨经纤维支气管镜支气管肺泡灌洗治疗重症肺部感染的临床疗效和安全性.方法 对本院住院诊断为重症肺部感染的54例患者,随机分组,治疗组纤支镜支气管肺泡灌洗联合常规抗感染、对症支持、呼吸机辅助通气,对照组用常规抗感染、对症支持、呼吸机辅助通气治疗.结果 治疗组在撤机时间、体温恢复正常时间、症状消失时间、血象恢复正常时间、住院天数、血气改善情况均优于对照组,有统计学意义.结论 经纤支镜支气管肺泡灌洗联合常规治疗方法能迅速清除痰痂和气道分泌物,更快地改善症状,缩短住院时间,提高治愈率,安全性高.%Purpose To observe the clinical efficacy and securities of bronchoalveolar lavagn by fibrobronchescope in the treatment of severe pulmonary infections. Methods 54-patients who were diagnosed having severe pulmonary infections in our hospital were randomly divided into two treatment groups. The therapeutic group used a combination of bronchoalveolar lavage by fibronchoscope, routine systemic antibiotics medications and mechanical ventilation. The control group used routine systemic antibiotic medications and mechanical ventilation. Result The two groups were compared on the measures of mechanical ventilation time, hospital-stay time, temperature-recovery time, symptom-dispersal time, WBC recovery-time, blood gas analysis index. All comparisons were statistically significant.Conclusion Using a combination of bronchoalveolar lavage by bronchoscopes and routine systemic medications is safe, which can remove sputum blocking and airway secretions quickly, while the patients' symptoms can be improved more quickly and obviously, hospital-stay time can be shorten significantly, and curing rate can be enhanced.
