经纤维支气管镜肺泡灌洗治疗肺脓肿42例的临床疗效及治疗体会

摘要

Purpose To observe and understand the clinical efficacy of bronchescope in the treatment of pulmonary abscess.Methods 74 cases of pulmonary abscesses were randomly divided into two treatment groups,42 cases of therapeutic group,using a combination of local irrigations and medications by bronchoscopes and systemic medications,and 32 cases of control group,using rourine systemic medications only.Results In the therapeutic group,30 cases were cured,9 cases were improved,and 3 cases were not improved;in the control group, 16 cases were cured,7 cases were improved,8 cases were not improved, and 1 case died.Comparing with 2 treatment groups,curing rates and total effective rates were significantly different( P ＜ 0.05 ).Body-temperature normalizing time and treatment time were significantly shortened in the therapeutic group( P＜0.01 ).Conclusions Using a combination of local irrigations and medications by bronchoscopes and routine systemic medications,the treatment effectiveness of pulmonary abscess can be significantly enhanced and treatment time can be significantly shortened.This therapeutic method is worth to be popularized and used clinically.%目的 观察经纤支镜治疗肺脓肿的临床疗效及治疗体会.方法 将74例肺脓肿随机分为两组,治疗组42例,经纤支镜局部灌洗、局部注药联合内科治疗;对照组32例,予内科常规治疗.结果 治疗组治愈30例,好转9例,无效3例;对照组治愈16例,好转7例,无效8例,死亡1例.两组治愈率及总有效率比较,差异有显著性(P<0.05).治疗组在体温恢复时间及疗程较对照组明显缩短(P<0.01).结论 经纤支镜局部灌洗、局部注药联合内科常规治疗肺脓肿,能显著提高疗效,缩短疗程,值得临床推广应用.
