Objective To detect and analyze changes in the AECOPD patients with CRP before and after treatment, and to explore the clinical value of dynamic change. Method 42 patients with AECOPD were selected. CRP levels before treatment and 10 ~ 14 days after the use of antibiotics were tested. The relationships between CRP and leukocytes, the proportion of neutrophils were analyzed. Results CRP levels were significantly higher in patients with acute exacerbation of ( 57. 224 ± 54. 86552 mg/L ), and declined rapidly after treatment ( 10. 3976 ± 15. 1405 mg/L ) , and there was significant difference ( P < 0. 05 ). Moreover, the changes of CRP after treatment had significant relationship with the total number of leukocytes and the proportion of neutrophilssignificant. CRP levels were positively related with the prognosis, and the treatment course of anti-infection and length of stay lengthened with the increase of CRP in chronic obstructive pulmonary disease with acute exacerbation. Conclusion Detection of CRP may play an important role in the diagnosis, observation of treatment effect and prognosis. However, the following conditions must be considered: 1. the increases of CRP delay in a severe infection. 2. other factors such as cancer, tissue damage also make an increase of CRP. Therefore, the total number of leukocytes and the proportion of neutrophilssignificant should be applied combinedly in the clinical so as to reduce missed diagnosis and diagnostic errors.%目的 检测并分析AECOPD病人C反应蛋白治疗前后的变化,探讨其动态变化的临床价值.方法 选择42例AECOPD患者,检测其在治疗前及使用抗生素后10~14天的CRP水平,分析这些指标的变化情况,并分析其与白细胞总数及中性粒细胞比例的相关性.结果 在急性加重期CRP水平明显升高(57.224±54.86552 mg/L),而治疗后迅速下降(10.3976±15.1405 mg/L),其变化差异具有统计学意义,且与白细胞总数及中性粒细胞比例具有显著相关性,此外CRP与预后具有正相关,抗感染疗程及住院时间有随CRP增高而延长趋势.结论 CRP检测对COPD急性加重期感染的诊断及疗效观察、预后判定具有一定的辅助诊断意义.
