白内障超声乳化吸出术中应用多孔灌注前房维持器的临床价值

摘要

Objective To study the clinical value of porous perfusion anterior chamber maintainer (ACM) in phacoemulsification.Methods 80 age-related cataract patients admitted into our hospital between January 2015 and August 2016 were selected as research subjects and were treated with phacoemulsification.40 patients usedviscoelastics during the surgery (control) and 40 patients used ACM.Surgery duration,phacoemulsification duration,phacoemulsification energy,visual acuity before and after surgery,intraocular pressure (IOP),corneal endothelial cell (CEC) count,loss rate of endothelial cell,variable coefficient of corneal endothelial cell area,central corneal thickness (CCT),corneal astigmatism,surgically induced astigmatism (SIA) level of the two groups were compared.Results There was no difference in the average surgery time,phacoemulsification time,phacoemulsification energy between two groups (P ＞ 0.05);3 months after surgery,the visual level,loss rate of endothelial cell,variable coefficient of corneal endothelial cell area,CCT,astigmatism in two groups were higher than pre-surgery,the IOP,CEC count,SIA were lower than pre-surgery (P ＜ 0.05),but there was no difference in the above indexes between two group(P ＞ 0.05).Conclusions Porous perfusion ACM in phacoemulsification has high clinical values.It can improve visual acuity,control IOP,reduce corneal endothelial cell damage and facilitate visual recovery on the basis of maintaining anterior-chamber dynamic stability.%目的 研究白内障超声乳化吸出术中应用多孔灌注前房维持器(ACM)的临床价值.方法 选取我院2015年1月至2016年8月收治的80例(80只眼)年龄相关性白内障患者作为研究对象,均采用白内障超声乳化吸出术治疗.将手术中应用黏弹剂的40例患者纳入对照组,应用多孔灌注前房维持器的40例患者纳入研究组.比较两组患者手术时间、超声乳化时间、超声乳化能量、手术前后视力水平、眼压、角膜内皮细胞计数、内皮细胞丢失率、角膜内皮细胞面积变异系数、中央角膜厚度、角膜散光、术源性散光水平.结果 两组患者平均手术时间、平均超声乳化时间、平均超乳能量比较,差异无统计学意义(P＞0.05);术后3个月,两组患者视力水平、内皮细胞丢失率、角膜内皮细胞面积变异系数、中央角膜厚度、角膜散光水平均高于术前,术后3个月两组患者眼压水平、角膜内皮细胞计数、术源性散光水平均低于术前,差异有统计学意义(P＜0.05),但两组间上述指标比较,差异均无统计学意义(P＞0.05).结论 在白内障超声乳化吸出术中应用多孔灌注前房维持器临床价值高,能在维持前房动态稳定基础上,提高患者视力水平,调节眼压,减轻角膜内皮细胞损伤,利于视觉功能恢复.
