儿童先天性白内障双微创联合手术临床研究

摘要

Objective To evaluate the efficacy of 1.8 mm corneal micro-incision phacoerysis combined with 25+Gauge vitrectomy for the treatment of pediatric congenital cataract.Methods In this retrospective interventional study of 21 cases 30 eyes aged from 2 to 11 years with congenital cataract were included to undergo 1.8 mm micro-incision phaco-erysis,combined with intraocular lens implantation, posterior capsular incision and limited anterior vitrectomy using a 25-gauge vitrectomy system.All cases were followed 6-12 months.Vision acuity and post-operative complications like position of intraocular lens,posterior capsular opacity,synechiae and the intraocular pressure were observed.Results all opera-tions were successful.The anterior chamber stabled very smooth and better control in operation.Less Inflammatory response and Surgically induced astigmatism after surgery, and better improve vision post-operation.The complications were not found in all cases.Conclusions The combined double microtrumatic operation for pediatric congenital cataract is a safe and effective approach.%目的 评价1.8 mm透明角膜微切口晶状体针吸联合25G+微创玻璃体切除手术系统应用于儿童先天性白内障的效果.方法 回顾分析21例(30只眼)2~11岁先天性白内障患儿的临床资料.患者均分别行1.8 mm透明角膜微切口晶状体针吸+人工晶状体植入+25 G晶状体后囊切除+前段玻璃体切除双微创联合手术.观察术中情况,术后视力、人工晶状体位置、后囊膜混浊、虹膜粘连及眼压等情况,随访6~12个月.结果 全部手术均顺利,术中前房稳定、操作控制性好,术后炎症反应轻、术源性散光小、视力明显改善,无虹膜后粘连、人工晶状体偏位或后囊膜混浊等并发症.结论 双微创联合手术应用于儿童先天性白内障,对眼表和眼内创伤轻微、无明显并发症、安全有效.
