目的 观察腹腔镜左肝外叶切除术治疗肝内胆管结石的临床疗效,总结与开腹手术相比的优劣.方法 选择2008年3月-2011年3月本院接受左肝外叶切除术治疗肝内胆管结石的71例患者,按照手术途径的不同分为腹腔镜组和开腹组,对比2组临床疗效.结果 2组患者手术时间、术中出血量、术后并发症以及医药费用等方面无显著差异,腹腔镜组患者的术后恢复情况显著优于开腹组,切口总长度显著短于开腹组,手术均获成功,无死亡病例,随访结果令人满意.结论 腹腔镜左肝外叶切除术治疗肝内胆管结石疗效确切,且具有创伤小,恢复快等优势.%Objective To observe the clinical efficacy between laparoscopy and lateral lobe of left liver resection for hepatolithiasis, and summarize the advantages and disadvantages. MethodsrnSeventy-one patients with lateral lobe of left liver resection for hepatolithiasis from March 2008 to March 2011 were divided into the laparoscopy group and laparotomy group. The clinical effects of the two groups of patients were compared. Results The surgical time, bleeding amount, postoperative complications, and medical expenses between the two groups showed no significant difference. The postoperative recovery condition in the laparoscopy group was superior to that in the laparotomy group, and the incision length was significantly shorter than that in the laparotomy group. All of the patients achieved success, and there was no death case. The results are satisfactory. ConclusionrnLateral lobe of left liver resection by laparoscopy has correct efficacy in the treatment of hepatolithiasis and with the advantage of less trauma and quicker recovery.
