目的 探讨锁骨钩钢板治疗NeerⅡ型锁骨远端骨折及TossyⅢ型肩锁关节脱位的临床疗效和可能发生的并发症.方法 对2007年2月～2012年2月收治的50例新鲜TossyⅢ型肩锁关节脱位和12例新鲜NeerⅡ型锁骨远端骨折患者进行切开复位锁骨钩钢板内固定手术治疗,术中常规修复断裂的喙锁韧带.结果 术后随访12～36个月,平均15个月,术后X线片复查,12例骨折患者全部骨性愈合,50例肩锁关节脱位患者未出现再脱位.术后功能优54例,良5例,差3例,优良率95.16％.结论 采用锁骨钩钢板治疗肩锁关节脱位和锁骨运端骨折固定确切,可早期行肩关节功能锻炼,术中应常规修复喙锁韧带及肩锁韧带,可获得满意的临床疗效.%Objective To explore the clinical efficacy and potential complications of clavicular hook plate in the treatment of Neer II acromioclavicular joint dislocation and Tossy III distal clavicular fracture. Methods Fifty patients with Tossy III acromioclavicular joint dislocation and 12 Neer II distal clavicle fracture who were admitted to our hospital from February 2007 to February 2012 received open reduction and internal fixation with clavicular hook plate. The fractured coraco-clavicular ligament during the operation was routinely repaired. Results The postoperative follow - up ranged from 12 to 36 months with an average of 15 months. All the 12 fractured patients obtained bone union shown by X- ray examination. The 50 patients with acromioclavicular joint dislocation did not relapse. Function examination showed that 54 cases were excellent,5 cases were good and 3 cases were poor after the operation. The rate of excellent and good results was 95.16% . Conclusion The application of clavicular hook plate in the treatment of complete acromioclavicular joint dislocation and distal clavicle fracture has confirmed outcomes. The functional exercise of shoulder joint can be done early. The fractured coracoclavicular ligament should be routinely repaired. By doing these,satisfactory clinical efficacy be can obtained.
