腹腔镜阑尾切除术中阑尾根部三种处理方法的对比分析

摘要

Objective To investigate the clinical effect of 3 approaches for securing appendicular stump during laparoscopic appendectomy.Methods 120 cases patients with acute appendicitis admitted to the operation were selected from 2011 March to March 2014, and divided into A group(silk ligation), B group (Hem-o-lok clipping method), C group(Group Endoloop ligation), 40 cases of each group. All the patients underwent laparoscopic appendectomy, and then comparision the clinical effects of the three groups.Results All the patients were all successfully completed the operation, no conversion treatment. Operation time of A group, B group, C group were(55.32+ 10.21)min,(40.32+ 8.32)min,(50.39+ 9.55)min, and the treatment costs were(13 092.76+ 24.32) yuan,(7 808.76+ 23.21)yuan,(8 923.21+ 25.21)yuan, visible in the B group, operation the time and cost of treatment were less than the other two methods (P<0.05).Conclusion The three kinds methods are safe, effective, but in improving operation speed, saving the patient treatment costs, Hem-0-lok clipping method is better than the other two methods, which is worthy of clinical use.%目的：探讨腹腔镜阑尾切除术阑尾根部3种处理方法的临床效果情况。方法选取2011年3月～2014年3月收治的需要进行手术的急性阑尾炎患者120例，根据就诊序号随机分组方法分成A组、B组、C组，各40例，全部患者均进行腹腔镜阑尾切除术，A组给予丝线结扎方式处理阑尾根部，B组给予Hem-o-lok夹闭方法处理阑尾根部，C组给予Endoloop套扎方法处理阑尾根部，比较三组患者的临床治疗情况。结果所有患者全部顺利完成手术，无中转开腹治疗。A组、B组、C组的手术时间分别为（55.32±10.21）min、（40.32±8.32）min、（50.39±9.55）min，治疗花费分别为（13092.76±24.32）元、（7808.76±23.21）元、（8923.21±25.21）元，可见B组在手术时间和治疗花费上较其他两种方法具有显著性优势（P＜0.05）。结论三种阑尾根部处理方法均具有良好的安全性、有效性，但在提高手术速度、节约患者治疗费用方面Hem-o-lok夹闭方法较其他两种方法具有明显优势。
