目的 探讨聚焦超声辐照豚鼠慢性湿疹模型豚鼠的皮肤病理结构改变.方法 豚鼠50只,以2,4-二硝基氯苯为抗原,小剂量多次刺激豚鼠背部皮肤制备慢性湿疹模型.将模型随机分为1W组、2W组、3W组和对照组,分别于辐照后7d、14 d取皮肤组织,进行光镜检查,3W组做电镜观察.结果 湿疹模型表皮棘层肥厚,细胞间轻度水肿,真皮大量炎性细胞浸润.超声辐照后7d充血水肿消退,棘层肥厚减轻,真皮炎性细胞浸润减少,辐照后14d真皮浅层少量炎性细胞浸润,部分胶原纤维增生,其余基本正常.对照组7d和14 d时炎症仍明显.超微结构变化:模型组可见表皮角质层增厚明显,成纤维细胞线粒体肿胀、粗面内质网扩张,细胞间隙加大,辐照后7d无明显变化,仅细胞间隙变小,14 d恢复正常.结论 聚焦超声可用于治疗湿疹,有利于皮肤炎症的恢复和组织的修复.%Objective To investigate dermatopathological changes of guinea pig model of chronic eczema after treated by focused ultrasound. Methods The chronic eczema models were prepared by using 2,4-dinitrochlorobenzene (DNCB) as an antigen to stimulate repeatedly the shaved sites on the back of guinea pigs. 50 guinea pigs were randomly divided into 1 W group,2 W group,3 W group,and control group. Skin tissue samples were taken on 7th and 14th days after ultrasound irradiation, respectively. Samples were examined by light microscope, and the 3 W group was observed with electron microscope. Results There were epidermal acanthosis of the model of eczema epidermal acanthosis,slight intercellular edema,and large of inflammatory cells infiltration in dermis. On 7th day after ultrasound irradiation,there were significant decreased of the edema subsided,and dermal infiltration of inflammatory cells decreased. On 14th day after ultrasound irradiation a small amount of inflammatory cells infiltrated in superficial dermis was seen,some collagenous fiber proliferated was noticed,the others was basically normal. In control group inflammation was still obvious on 7th day and 14th day after ultrasound irradiation. Ultrastruc-tural changes: In the model group, the visible stratum comeum thickened apparently. The mitochondria of fibroblast swelled. The rough endo-plasmic reticulum expanded and the intercellular space increased. On the 7th day after irradiation no significant changes was found,and merely intercellular space became smaller. On the 14th day it returned to normal. Conclusion Focused ultrasound could be used to treat eczema and it was beneficial to the recovery of skin inflammation and tissue repair.
