目的 探讨长程视频脑电图(V-EEG)在癫痫患者规范化诊断及治疗中的应用.方法 对500例患者进行24小时V-EEG记录,明确癫痫诊断并分类,进行规范化治疗.结果 500例中大致正常脑电图93例；轻度及可疑异常脑电图42例；异常脑电图365例,其中癫痫综合征82例.痫样放电以睡眠Ⅰ、Ⅱ期最著,前颞区最多.共监测到临床发作178例,排除癫痫18例,确诊癫痫160例,其中部分性发作130例,以复杂部分性比例最高；全面性发作30例.接受规范药物治疗285例；外科手术治疗80例,其中颅内埋藏电极10例.结论 无论是在癫痫诊断、鉴别诊断、正确分型、合理选药,还是在术前定位致痫灶、评价治疗效果及选择减停药时机方面,V-EEG起到了其他检查不可替代的作用,对癫痫的规范化诊疗具有重要意义.%Objective To explore the application of long term video-electroen- cephalogram ( V-EEG) momitoring in people with epilepsy diagnosis and treatment of standardization. Methods 500 patients with 24 hours or more records, clear epilepsy diagnosis and classification, carried out standard treatment. Results Results In 500 cases, 93 cases were roughly normal EEG; 42 cases were mild and suspicious abnormal; 365 cases were abnormal EEG. 82 cases of epilepsy syndrome. Epilepsy discharge was the most of NREM Ⅰ ,Ⅱ term and the temporal region . Monitoring to 178 cases of clinical seizure, 18 cases eliminate epilepsy, 160 cases identified epilepsy seizure,and 130 cases was part seizure, with the highest percentage of complex part seizure. 30 cases was Comprehensive seizure. Accepted the standard drugs of 285 cases. Surgical treatment for 80 cases, 10 cases of intracranial buried electrode among them. Conclusion Whether in epilepsy diagnosis, differential diagnosis, the correct classification, reasonable choose medicine, or in the preoperative epileptogenic zone localization, evaluate the efficacy of the treatment and choose stop drug reduced time, V-EEG plays an important role that the other check cannot be replaced, having the important meaning to the standardization of epilepsy diagnosis and treatment.
