电子处方管理办法的实施对门诊处方质量影响的研究

摘要

Objective:Explore the effect of implementation of prescription management method of electrons on the quality of outpatient prescriptions in our hospital. Methods:A random sample without the use of electronic prescription system(2008 January to June) of the 3 000 prescriptions and the use of electronic prescription system(2009 January to June) of the 3 000 prescriptions,according to the drug instructions,"prescription management method","prescription review implementation specification( Trial)" and other statistical analysis was carried out, the main contents include:analysis whether the standard of prescription writing, clinical medication is reasonable and the use of antibacterial drugs. Results:Without the use of the measures for the administration of electronic prescription, non normative prescriptions accounted for 22. 27%,the rate of irrational drug use in 11. 20%,the usage rate of antibiotics was 60. 3%, after the implementation of the measures for the administration of electronic prescription,non normative prescriptions accounted for 2. 43%,the rate of irrational drug use was 1. 26%,anti bacteria drug utilization rate was 39. 4%,with a significant difference in the quality of the use of electronic prescriptions(P<0. 01). Conclusions:The prescription management method of implementing electronic hospital outpatient prescription writing, can significantly improve the normative, rational clinical use of drugs, improve the quality of prescription,has obvious clinical significance.%目的：：探讨电子处方管理办法的实施对门诊处方质量的影响。方法：随机抽取未使用电子处方系统(2008年1~6月)的处方3000张和使用电子处方系统(2009年1~6月)的处方3000张，根据药物说明书、《处方管理办法》《处方点评实施规范(试行)》等分析处方书写是否规范、临床用药是否合理及抗菌药物使用情况。结果：未使用电子处方管理办法前，不规范处方率为22.27%，不合理用药率为11.20%，抗菌药物使用率为60.3%，实施电子处方管理办法后，不规范处方率为2.43%，不合理用药为1.26%，抗菌药物使用率为39.4%，使用电子处方前、后质量差异均有统计学意义(P<0.01)。结论：医院门诊实施电子处方管理办法，可显著改善处方书写的规范性、临床合理用药情况等，对提高处方质量具有较大的临床意义。
