逆向穿刺法在腹腔镜完全腹膜外疝修补术中的应用

摘要

Objective To explore the application of reverse puncture technique in laparoscopic complete peritoneal hernia repair (TEP).Methods From July 2016 to July 2017,of the patients undergoing TEP in the Department of Gastrointestinal Surgery of Kunshan First People's Hospital,15 cases were chosen,and the retroperitoneal space was established by reverse puncture method.This method was compared with traditional mirror push TEP.Results In all 15 cases using reverse puncture method to establish the retroperitoneal space,the operations were successful.The TEP with reverse puncture method had some advantages.Conclusions It is feasible to operate TEP with reverse puncture method,which has certain advantages in some patients.%目的 探讨采用逆向穿刺法在腹腔镜完全腹膜外疝修补术(totally extraperitongeal,TEP)中的应用体会.方法 自2016年7月至2017年7月期间所有在昆山市第一人民医院胃肠外科行TEP的病人中,选取15例病人采用逆向穿刺法建立腹膜前间隙,并与传统镜推法TEP进行对比总结.结果 所有15例采用逆向穿刺法建立腹膜前间隙的病人全部手术成功,与传统TEP手术相比,腹膜前间隙建立时间、疝囊剥离时间以及旋转补片时间均明显缩短(均P＜0.05).结论 对于身材矮小需要置入大补片的病人,以及疝囊难以剥离的病人,采用逆向穿刺法行TEP手术具有一定的优势.
