快速康复对腹腔镜辅助阴式全子宫切除术患者的临床探讨

         

摘要

目的:探讨快速康复外科(FTS)理念对妇科腹腔镜辅助阴式全子宫切除术患者康复进程的影响。方法:选取因子宫良性疾病行腹腔镜辅助阴式全子宫切除术(LAVH)的患者65例,对照组30例行 LAVH 术常规处理,研究组35例予 FTS 理念下的优化处理;比较两组患者术后皮质醇、C -反应蛋白的应激生化指标,及术后肛门恢复排气时间、并发症发生率、术后住院时间的临床指标。结果:两组手术均顺利完成,无明显手术并发症。在术后皮质醇、C -反应蛋白、肛门排气时间、术后住院时间,研究组分别是(399.80±49.83)nmol/ L、(22.11±3.66)mmol/ L、(22.54±3.02)h、(3.80±0.53)d;对照组分别是(494.33±45.39)nmol/ L、(34.10±6.07)mmol/ L、(29.00±3.90)h、(5.17±0.53)d,差异均有统计学意义(P <0.05)。结论:快速康复优化措施的应用可加快妇科 LAVH 患者术后的康复进程,降低手术应激,减少并发症的发生、缩短住院时间。%Objective To investigate the fast track surgery( FTS)effect of aginal hysterectomy patients in rehabilitation process concept in gynecological laparoscopic assisted. Method 65 cases of patients for uterine benign diseases underwent lapa-roscopic assisted vaginal hysterectomy(LAVH)were selected,30 cases underwent conventional treatment of LAVH operation in the control group,in the study group 35 cases were treated with FTS under the principle of optimization of treatment;patients were compared between the two groups after cortisol,C - reaction protein,and postoperative anal exhaust time,hospitalization time re-covery rate of complications,postoperative clinical indicators. Results The two groups of patients were successfully completed, without obvious complications. The hospitalization time,postoperative cortisol in C - reaction protein,anus exhaust time,postoper-ative,study group were(399. 80 ± 49. 83)nmol/ L,(22. 11 ± 3. 66)mmol/ L,(22. 54 ± 3. 02)h,(3. 80 ± 0. 53)d,respective-ly;the control group(494. 33 ± 45. 39)nmol/ L,(34. 10 ± 6. 07)mmol/ L,(29. 00 ± 3. 90)h,(5. 17 ± 0. 53)d,respectively, the difference was statistically significant(P < 0. 05). Conclusion The application of fast recovery optimization measures can speed up the rehabilitation process of patients after gynecological surgery LAVH,reduce stress,reduce complications occur,short-en the hospitalization time.
