正畸力作用下人龈沟液内基质金属蛋白酶-2及金属蛋白酶组织抑制因子-2的水平变化及意义

         

摘要

目的:探讨不同正畸力作用下人龈沟液(GCF)内基质金属蛋白酶-2(MMP-2)及金属蛋白酶组织抑制因子-2(TIMP-2)水平的变化及其临床意义。方法选择新余市人民医院口腔科就诊接受正畸治疗的患者32例,根据远中移动的初始力随机分成2组(A和B组),每组16例,分别施加100g、250g的远中移动的初始力,在加力前0周及加力后1、2、4、6、8周分别收集患者GCF,采用ELISA方法测定GCF中MMP-2及TIMP-2的含量。结果 A组患者GCF中MMP-2和TIMP-2浓度在加力后均持续增高,于4周时升至高峰,6周开始逐渐下降,8周恢复至基线水平。 B组患者GCF中MMP-2浓度在加力前后未出现明显变化,差异无统计学意义(P>0.05);TIMP-2的浓度在加力后1周时已出现显著增高,于4周时升至高峰,6至8周持续维持在高水平。 A组加力后各时间点GCF中MMP-2浓度明显高于B组(P<0.05);而TIMP-2的浓度则明显低于B组(P<0.05)。结论 MMP-2及TIMP-2参与了早期正畸牙周组织改建,且对维持牙周组织健康及正畸牙的移动速度可能起重要作用。%Objective To study the dynamic change of MMP-2 and TIMP-2 levels in gingival crevicular fluid (GCF) of human during orthodontic tooth movement under orthodontic forces,explore the role in orthodontic tooth movement and the influence of different strength of its expression. Methods 32 orthodontic patients in outpatient department (aged 13-20 years)were chosen in this study. The cases were divided into two groups (A and B) by force at random:100g and 250g groups,16 patients per group. The mechanical forces were exerted to the canine tooth. The GCF samples were collected at 0,1,2,4,6 and 8 weeks respectively after the force was applied. The levels of MMP-2 and TIMP-2 in GCF were determined by ELISA. Results The levels of MMP-2 and TIMP-2 in A group began to increase at 1 and 2 weeks,reached the peak level when after 4 weeks,it became to fall 6 later,final-ly returned to the beginning level at 8 weeks;The levels of TIMP-2 in A group began to increase at 1,2,4 week and decreased at 6 week after initiation of the experiment,but the level at 8 week was higher than that before initiation of the experiment. The level of MMP-2 in A group was higher than that in B group at all times and the level of TIMP-2 in A group was lower than that in B group(P<0.05). Conclusion MMP-2 and TIMP-2 in the early orthodontic periodontal tissue reconstruction,and to help maintain a healthy periodontal tissue and orthodontic tooth movement speed may play an important role.
