静脉输注硫酸镁对腹腔镜胆囊切除手术应激反应的影响

摘要

目的 观察硫酸镁对手术应激反应的影响.方法 40例择期在全身麻醉下行腹腔镜胆囊切除术(LC)患者随机分为硫酸镁(MgSO4)组和生理盐水(NS)组,每组20例.MgSO4组于麻醉前30min静脉输注MgSO4 50mg·kg-1·h-1至手术结束;NS组以生理盐水代替MgSO4.记录麻醉诱导前、气腹期及拔管时BP和HR的变化.采血测定血清皮质醇(Cor)、醛固酮(Ald)和血糖(BG)水平.结果 与麻醉前比较,术中两组HR、BP、Cor、Ald和BG均明显升高(P<0.05),但MgSO4组低于NS组(P<0.05).结论 静脉输注硫酸镁能在一定程度上抑制腹腔镜胆囊切除术中的应激反应.%Objective To observe the effects of magnesium sulfate (MgSO4) infusion on stress in patients undergoing laparoscopic cholecystectomy(LC). Methods Fourty LC patients under general anesthesia were randmly divited into two groups with 20 cases each. MgSO4 50 mg · kg-1 · h-1 was infused at 30 min before anesthesia till the end of surgery in group Mg. Normal saline was used instead of MgSO4 as the control in group NS. Before anesthesia induction, during pneumoperitonium and at the time of extubation, BP and HR were recorded and serum cortisol(Cor), aldosterol(Ald) and blood sugar(BG) were measured. Results Compared to before, HR, BP, Cor, Ald and BG in both groups were all increased during operation, which were lower in group Mg than those in group NS(P＜ 0. 05). Conclusion The stress responses to LC under general anesthesia may be attenuated by intravenous infusion of MgSO4 during operation.
