甲状腺疾病再次手术并发症的Meta分析

摘要

目的 分析甲状腺再次手术并发症发生情况.方法 分析57例甲状腺再次手术的术后并发症发生情况.检索万方数据库,得到22篇甲状腺初次手术并发症和43篇甲状腺再次手术并发症论文的原始资料,进行Meta分析.结果 甲状腺再次手术的57例中,出血1例,右侧喉返神经损伤3例,并发症发生率7.02％.Meta分析结果:再次手术的喉返神经损伤、甲状腺功能低下、甲状旁腺功能低下和出血的发生率分别为5.40％、4.06％、9.61％和0.65％,高于首次手术者的0.70％、0.44％、0.50％和0.24％(P＜0.01).甲状腺再次手术并发症的发生率远高于首次手术(21.13％ vs.3.09％)(P＜0.01).结论 甲状腺疾病再次手术的术后并发症多,需要注意把握再次手术的时机.%Objective To analyze the complications of thyroid reoperation after thyroidectomy in the past Methods The complications of thyroid reoperation were summarized retrospectively in 57 cases who underwent thyroidectomy in the past A Meta analysis on the complications of thyroid reoperation after thyroidectomy was performed by searching the data from Wangfang database. Results The incidence rate of complications in 57 cases received thyroid reoperation was 7. 02% (4/57) including hemorrhage in one case and recurrent laryngeal nerve injury on the right side in 3 cases. Meta analysis showed that the incidence rate of complications was higher in the cases underwent thyroid reoperation than that in those during the first operation (21. 13% vs. 3.09%) (P<0. 01). The incidence rates of laryngeal nerve injury, hypothyroidism, hypoparathyroidism and hemorrhage were 5. 40%,4. 06%,9. 61% and 0. 65%,respectively,summarized from 22 papers of thyroidectomy for the first time,which were higher than 0. 70%,0.44%,0. 50% and 0. 24%,respectively,summarized from 43 papers of thyroid reoperation(P<0. 01). Conclusion The incidence rate of complications in the patients undergoing thyroid reoperation, who underwent thyroidectomy in the past, is high and attention should be paied to the time selected for the second thyroid surgery.
