Objective To investigate the value of 68Ga somatostatin receptor imaging in the localization diagnosis of functional islet cell tumor.Methods The clinical data of 4 patients with functional islet cell tumor were retrospectively analyzed and the value of 68Ga somatostatin receptor imaging in the localization diagnosis of functional islet cell tumor was discussed.Results The resection of the body and tail of the pancreas was performed in the case one but severe clinical symptoms remained after operation.And then a tumor on the posterior pelvic wall was found by 68 Ga somatostatin receptor imaging,which was resected and confirmed pathologically to be islet cell tumor.One tumor in the body of the pancreas by CT scan in the case two was reported.Another tumor was showed by 68Ga somatostatin receptor imaging on the lower border of the pancreas,which was present after reading the CT film again carefully.The two tumors were resected and confirmed pathologically to be islet cell tumors.A mild fortified foci in the body of the pancreas was found by abdominal CT scan in the case three,which could not be identified but was confirmed afterwards by 68Ga somatostatin receptor imaging and confirmed pathologically to be islet cell tumor after operation.A tumor in the case four was reported by abdominal CT scan,which was failed to be shown by 68Ga somatostatin receptor imaging.The tumor was removed and confirmed pathologically to be islet cell tumor after operation.The islet cell tumors were all resected successfully and cured after operation in all 4 cases.Conclusion 68Ga somatostatin receptor imaging has an important value in the localization diagnosis of functional islet cell tumor,especially in the cases with the tumor that can not be identified by CT scan or the tumor is ectopic or multiple.But false negative result for the functional islet cell tumor may be reported by 68Ga somatostatin receptor imaging examination and it is necessary to be combined with other imaging methods in order to avoid omission of diagnosis.%目的 探讨68Ga生长抑素受体显像在功能性胰岛细胞瘤定位诊断中的价值.方法 回顾性分析4例功能性胰岛细胞瘤患者的临床资料,讨论68Ga生长抑素受体显像在功能性胰岛细胞瘤定位诊断中的价值.结果 例1腹部CT检查未发现瘤体并进行剖腹探查胰体尾部切除,但术后严重症状仍然存在;后经68Ga生长抑素受体显像发现盆底后壁肿块,再经盆腔CT检查进一步证实并再次手术切除胰岛细胞瘤.例2 CT报告为胰体部1个瘤体,经68Ga生长抑素受体显像胰腺下缘另1个瘤体;再次仔细阅CT片确定有2个瘤体,经手术切除病理证实是胰岛细胞瘤.例3腹部CT检查胰体部轻度强化灶,不能明确诊断;后经68 Ga生长抑素受体显像显示为1个瘤体,经手术切除病理证实为胰岛细胞瘤.例4 CT显示胰尾部瘤体,而68 Ga生长抑素受体显像检查未能显示肿瘤;术后病理诊断为胰岛细胞瘤.4例患者均经手术治愈.结论 68 Ga生长抑素受体显像对CT不能明确、异位和多发功能性胰岛细胞瘤的定位诊断有重要的价值,但也存在假阴性,需联合其他影像学检查,以免漏诊.
