妊娠期糖尿病患者营养治疗的疗效观察

摘要

目的 观察妊娠期糖尿病患者营养治疗的疗效,了解其依从性及对分娩结局的影响.方法 将2012年1至7月来我院就诊的60例已经行糖耐量试验(OGTT)检测诊断为妊娠期糖尿病的患者随机分为交换法组与替代组,比较两组患者经过营养治疗后营养状况和临床治疗结果的差异,并对分娩结局进行比较.结果 替代组在短期内血糖下降时间优于交换法组(P＜0.05),在妊娠结局中合并羊水过多、胎膜早破、巨大儿、新生儿窒息、新生儿低血糖的发生率两组间差异无统计学意义(P＞0.05).尿酮体阳性发生率比较两组差异具有统计学意义(P＜0.05).依从性比较两组间差异无统计学意义(P＞0.05).结论 替代组短期的血糖控制效果优于交换法组,并能有效干预不良妊娠结局的发生.%Objective To observe the efficacy of nutrition therapy for gestational diabetes mellitus,to explore patient compliance and the impact on childbirth outcomes.Methods 60 women diagnosed as gestational diabetes by OGTT in our hospital during the period of January to July were randomly divided into a exchange group and a replacement group.The nutritional status and clinical efficacy were compared between the two groups after nutrition therapy,and the childbirth outcomes were also compared.Results The replacement group was better than the exchange group in time to a short-term decline in blood sugar (P ＜0.05).The rates of polyhydramnios,premature rupture of membranes,huge infants,neonatal asphyxia,and neonatal hypoglycemia did not differ statistically between the two groups.(P＞ 0.05),but there was statistical difference in the incidence of positive urine ketone bodies between the two groups (P ＜0.05).Patient compliance did not differ significantly between the two groups (P＞0.05).Conclusions The replacerment is superior to the excahnge group in short-term glycemic control,and can effectively intervent in the incidence of adverse pregnancy outcomes.
