后腹腔镜输尿管切开取石术治疗输尿管结石的疗效及并发症观察

摘要

Objective To discuss the curative effect and complications of retroperitoneoscopic ureterolithotomy (RIU) in the treatment of ureteral calculi.Methods 80 patients with calculi in the upper part of ureter treated in our hospital were selected as the study objects,and randomly divided into control group and observation group with 40 cases in each group.Patients in observation group were given RIU,while patients in control group were given ureteral lithotomy (UL).Recorded operation time,the amount of bleeding during operation,gastrointestinal functional recovery time,off-bed time,and the hospital stay of two groups.Meanwhile,recorded the calculi clearance rate 1 month after operation,and compared postoperative complications occurrence rate.Results 1 month after operation,the calculi clearance rate in observation group was 97.5％,and 92.5％ in control group,without statistically significant difference between two groups (P＞0.05).The amount of bleeding during operation,gastrointestinal functional recovery time,off-bed time,and the hospital stay in observation group were (24.19±18.92)ml,(2.11±0.36)d,(2.36±0.28)d,and (6.90±0.46)d respectively,obviously less than those in control group.Postoperative VAS score in observation group was (1.29±0.31),obviously lower than that in control group,with statistically significant difference (P＜0.05).Postoperative complications occurrence rate in observation group was 10.0％,obviously lower than 27.5％ in control group,with statistically significant difference (P＜0.05).Conclusion RIU is safe and effective in the treatment ofureteral calculi.%目的 探讨后腹腔镜输尿管切开取石术(RIU)治疗输尿管结石的疗效及并发症.方法 选取医院收治输尿管上段结石患者80例作为研究对象,采用数字随机对照表法将患者分为对照组和观察组各40例,观察组行后腹腔镜输尿管切开取石术(RIU),对照组行开放式输尿管切开取石术(UL),记录两组手术时间、术中出血量、胃肠功能恢复时间、下床活动时间及术后住院时间,记录两组术后1个月结石清除率,对比两组术后并发症发生率.结果 术后1个月,观察组39例结石清除,占97.5％;对照组37例结石清除,占92.5％;差异无统计学意义(P＞0.05).观察组术后出血量、胃肠功能恢复时间、下床活动时间、住院时间分别为(24.19±18.92) ml、(2.11±0.36)d、(2.36±0.28)d、(6.90±0.46)d,显著短于对照组,观察组术后VAS评分为(1.29±0.31),显著低于对照组,差异有统计学意义(P＜0.05).观察组术后并发症发生率为10.0％,显著低于对照组的27.5％,差异有统计学意义(P＜0.05).结论 RIU治疗输尿管结石安全有效.
