内皮微颗粒CD144+/Annexin V+水平与冠状动脉病变程度的相关性研究

摘要

目的 探讨内皮微颗粒CD144+/Annexin V+水平变化与冠状动脉病变程度的相关意义.方法 选取本院住院接受冠脉造影检查患者185例,根据造影结果分为冠状动脉病变组和冠脉正常对照组,冠脉病变组分为急性冠脉综合征(ACS)组和非急性冠脉综合征(非ACS)组,Gensini评分将冠脉病变患者分为低分值组(1.5 ~ 24)和高分值组(>24).流式细胞仪检测血浆内皮微颗粒水平,同时测定血浆胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、尿酸(UA)等相关指标进行统计学分析.结果 冠脉病变组尿酸明显高于健康对照组,高密度脂蛋白胆固醇则低于对照组(P<0.05);冠脉病变组血浆EMP-CD144+/Annexin V+高于对照组[1 940.86(2 519.79)个/μ1比1 228.32(1 671.89)个/μl;P=0.001];急性冠脉综合征患者EMP-CD 144+/Annexin V+明显高于非急性冠脉综合征患者[2 729.16(3 476.04)个/μ1比1 422.28(2 362.10)个/μl;P=0.001];Gensini评分高分值组EMP水平虽然高于低分值组,但差异无统计学意义(P>0.05).EMP-CD144+/Annexin V+水平与Gensini积分之间无线性相关关系.结论 血浆EMP-CD144+/Annexin V+与冠脉病变严重程度(即动脉硬化粥样斑块的大小)无相关性,但可能对冠状动脉粥样斑块的稳定性有一定影响.%Objective To investigate the relevance of levels of endothelial microparticles CD 144+/ Annexin V + and the severity of coronary artery lesions.Methods One hundred and eighty-five patients underwent coronary angiography in the department of cardiology in our hospital.According to the angiographic findings,2 groups were divided,such as the coronary artery lesion group and the normal control group.In the first group,there were 2 subgroups including acute coronary syndrome group (ACS),non-acute coronary syndrome group (non-ACS).Then,based on the Gensini score system,the coronary artery lesion group was divided into low score group (1.5-24) and high score group (>24).A flow cytometer detected the levels of plasma EMP-CD144+/Annexin V +,cholesterol,triglyceride,low density lipoprotein cholesterol,high density lipoprotein cholesterol,and uric acid for statistical analysis.Results Comparing with the normal control group,the coronary artery lesion group had high uric acid level (P < 0.05) and low high density lipoprotein cholesterol (P < 0.05);the levels of plasma EMP-CD144+/Annexin V + were 1 940.86 (2 519.79) (n/μl) in the coronary lesion group versus 1 228.32 (1 671.89) (n/μl) in the normal control group (P =0.001);the levels of plasma EMP-CD144+/Annexin V + were 2 729.16 (3 476.04) (n/μl) in the coronary lesion group versus 1 422.28 (2 362.1) (n/μl) in the normal control group (P =0.001).Between the low score group (1.5-24) and the high score group (>24) on Gensini score system,there was no significant difference in the level of EMPCD144+/Annexin V + and no linear correlation in the level of EMP-CD144+/Annexin V + (r =0.088,P-0.295).Conclusions Plasma levels of EMP-CD144+/Annexin V + does not correlate with the severity of coronary artery lesions,but it may impact on the stability of the coronary atheromatous plaque.
