改良叩背护理法对脑出血患者肺部感染的干预效果观察

摘要

Objective To observe the intervention effect of improved knock back nursing method on pulmonary infection in patients with cerebral hemorrhage.Methods 80 cases of cerebral hemorrhage were divided into 2 groups according to the random number table method.40 cases in control group received the traditional knock back nursing method,while 40 cases in observation group received improved knock back nursing method.The blood gas index,sputum sticky degree,sputum volume,the incidence of pulmonary infection,and knock back comfort degree of the two groups before and after sputum excretion were compared.Results There were no statistically significant differences in blood gas indexes (PO2,PCO2,SaO2) between two groups before sputum excretion (P＞0.05),blood gas indexes in two groups significantly improved after sputum excretion,PO2,PCO2 in observation group improved more significantly (P＜0.05).The sputum sticky degree in observation group was significantly better than that in control group (P＜0.05),sputum volume was significantly higher than that in control group [(44.65±11.32)ml vs.(30.12±9.45)ml,P＜0.05],the incidence of pulmonary infection was significantly lower than that in control group (5.0％ vs.27.5％,P＜0.05),the knock back comfort degree was significantly better than that in control group [(8.22±1.52)points vs.(6.76±1.18)points,P＜0.05].Conclusion Improved knock back nursing method can promote the sputum excretion of patients with cerebral hemorrhage,reduce the incidence of pulmonary infection,significantly improve the knock back comfort degree.%目的 观察改良叩背护理法对脑出血患者肺部感染的干预效果.方法 80例脑出血患者根据随机数字表法分为2组,对照组40例采用传统叩背护理方法,观察组40例采用改良叩背护理方法.比较两组排痰前后血气指标、痰液黏稠程度、排痰量、肺部感染发生率及叩背舒适度.结果 两组排痰前PO2、SaO2、PCO2血气指标比较差异无统计学意义(P＞0.05),两组排痰后血气指标均明显改善,且观察组PO2、PCO2血气指标改善程度较对照组更为显著(P＜0.05).观察组痰液粘稠程度明显优于对照组(P＜0.05),排痰量明显多于对照组[(44.65±11.32)ml vs.(30.12±9.45)ml,P＜0.05],肺部感染发生率明显低于对照组(5.0％ vs.27.5％,P＜0.05),叩背舒适度明显好于对照组[(8.22±1.52)分vs.(6.76±1.18)分,P＜ 0.05].结论 改良叩背护理法可促进脑出血患者痰液排出,降低肺部感染发生率,明显提高叩背舒适度.
