毫火针、白癜风颗粒、窄谱中波紫外线三联疗法对白癜风患者外周血中的IL-17、IL-10、IL-22表达的影响

摘要

目的 探讨毫火针、白癜风颗粒、窄谱中波紫外线(NB-UVB)三联疗法对白癜风患者外周血中的白细胞介素IL-17、IL-10、IL-22表达的影响.方法 选择2015年1月10日至2017年12月1日来自阳江市人民医院皮肤科门诊的白癜风患者100例,按照随机数字表法分为两组,观察A组50例采用白癜风颗粒内服并结合窄谱中波紫外线光疗,观察B组50例则在观察A组基础上联合毫火针治疗,对照组为同期来自阳江市人民医院门诊正常健康体检人群50例,并比较各组的临床效果及差异性.结果 观察B组总有效率为98.00％,明显高于观察A组的80.00％,差异有统计学意义(x2=8.2737,P=0.0000).治疗12周后,观察A组与观察B组IL-17及IL-22水平均较治疗前明显降低(P＜0.05),且观察B组IL-17及IL-22水平较观察A组降低更为显著(t=3.854 1,P=0.000 0;t=5.623 6,P=0.000 0);观察A组与观察B组IL-10水平较治疗前明显升高(P＜0.05),且观察B组IL-10水平较观察A组升高更为显著(t=7.1805,P=0.0000).结论 运用毫火针、白癜风颗粒、窄谱中波紫外线(NB-UVB)三联疗法治疗白癜风具有显著临床疗效,有效促进皮损复色,改善中医症候,三联疗法作用可能与减轻机体炎性反应有关.%Objective To investigate the effect of milli fire needle,vitiligo granules,and narrow band ultraviolet B (NB-UVB) triple therapy on the expression of interleukin (IL)-17,IL-10,and IL-22 in the peripheral blood of patients with vitiligo.Methods A total of 100 patients with vitiligo from the Department of Dermatology,People's Hospital of Yangjiang City from January 10,2015 to December 1,2017 were selected and were divided into two groups according to the digital table method.50 patients in the observation group A were treated with vitiligo granules and NB-UVB,50 patients in the observation group B were treated with milti fire needle based on the observation group A.The normal control group consisted of 50 healthy people from the outpatient department of Yangjiang People's Hospital during the same period.The clinical parameters of the three groups were compared.Results The total response rate in the observation group B (98.00％) was significantly higher than that in the observation group A (80.00％) (x2=8.273 7,P=0.000 0).After 12 weeks of treatment,the levels of IL-17 and IL-22 in the observation group A,B significantly decreased (P＜0.05),those in the observation group B were even lower (t=3.854 1,P=0.000 0;t=5.623 6,P=0.000 0);the level of IL-10 in the observation group A,B significantly increased (P＜0.05),that in the observation group B was even higher (t=7.180 5,P=0.000 0).Conclusion Milli fire needle,vitiligo granules,and NB-UVB triple therapy has significant therapeutic effect in the treatment of vitiligo,effectively reduces the symptoms of skin lesions,improves symptoms of traditional Chinese medicine,which may be related to the reduction of inflammatory reactions.
