麦冬注射液对实验性大鼠腹膜透析并发腹膜炎的影响

         

摘要

目的:应用含麦冬注射液的普通腹透液,对正常、腹膜炎模型大鼠进行腹膜透析,观察其对腹腔防御机制的影响和透析并发腹膜炎的疗效.方法:按随机法将40只清洁级SD大鼠分为模型麦冬透析组(A组)、析模常规透型组(B组)、正常麦冬透析组(C组)、正常常规透析组(D组)4组,每组10只.模型组大鼠腹腔注入大肠杆菌造成腹膜炎模型.分别以含生药1g/ml麦冬注射液的腹透液和不含麦冬注射液的普通腹透液对模型和正常大鼠透析,透析持续7天,期间观察各组动物的精神状态、进食、大小便、有无烦躁、寒战等一般情况,透析结束后检测腹透液常规、腹透液细菌培养+茵落计数及大鼠腹腔巨噬细胞功能.结果:A组、C组及D组腹透液白细胞总数、中性粒细胞数均显着低于B组(P <0.05或P<0.01),而A、C、D组间差异无显著性;各组腹透液蛋白质浓度无明显差异(P>0.05);B组的巨噬细胞吞噬百分率及吞噬指数显著低于A组、C组及D组(P<0.05或P<0.0I);C组的吞噬百分率显著高于D组,而两组的吞噬指数则无显著性差异.结论:含麦冬注射液的腹膜透析液能减轻腹膜透析腹膜炎的发病程度,其主要机制可能通过免疫调节增强腹腔巨噬细胞吞噬功能,提高腹腔防御机制.同时,未见该剂量的麦冬注射液对腹腔有明显的刺激性.%Objective;To investigate the effect of routine dialysate( RD) added with Ophiopogon Injection on rats with peritoneal dialysis complicated with peritonitis and to study its defense mechansim. Methods; Forty clean SD rats were allocated to four groups: RD added with ophiopogon injection model group ( GroupA) , RD in model group (Group B) , ophiopogon injection in normal group ( Group C) and RD in normal group ( Group D). Rat models with peritonitis were established by intraperitoneal injection of Bacillus coli. The treatment course lasted 7 days. The mental state, appetite, urine and stool, and symptoms such as chill and restlessness were observed. After dialysis, routine examination and bacterial culture of peritoneal dialysate were carried out. The function of macrophyge in abdominal cavity was detected. Results: The number of white blood cell and neutrophil in dialysate were much lower in Group A, Group C and GroupD than those in Group B ( P < 0. 05 or P <0. 01). While the differences among Group A, Group C and Group D were not significant. No significant difference of protein concentration was found among the four groups ( P > 0. 05 ). Percentage of phygocytosis and phygocytic index of macrophyge were lower in Group B than those in Group A, Group C and Group D (P <0. 05 or P <0. 01). Percentage of phygocytosis was higher in Group C than that in Group D. While the difference phygocytic index between the two groups was not significant. Conclusion: RD added with ophiopogon injection can improve peritoneal dialysis complicated with peritonitis. Its mechanism may be related to the regulation of immune function and the increase of phygocytosis of macrophyge and defense function in abdominal cavity. No obvious stimulation is found in the study.
