目的：观察针刺肩髎、臂臑穴结合康复治疗脑卒中患者偏瘫肩的临床疗效。方法：符合纳入标准的脑卒中偏瘫肩患者60例随机分为治疗组和对照组，每组30例。两组均给予常规康复治疗（使用PNF运动模式和技术），治疗组加用针刺肩髎、臂臑穴疗法。采用神经功能康复疗效评估法评定神经功能缺损程度；Fugl－Meyer运动积分（ FMA）和日常生活活动量（ ADL）评分评定治疗前后上肢运动功能；视觉模拟评分（ VAS）的方法评定偏瘫肩患者的疼痛治疗效果。结果：治疗4周后，神经功能康复疗效方面治疗组总有效率为96．7％，对照组为86．7％。通过两组治疗前后FMA运动积分及ADL评分表明，在改善患者肩关节的运动功能方面两组均明显提高，差异具有统计学意义（P＜0．05），治疗组优于对照组。视觉模拟评分（VAS）方面治疗组为90．0％，对照组为76．7％。结论：针刺肩髎、臂臑穴结合康复治疗脑卒中患者偏瘫肩优于单纯常规康复治疗方法。%Objective:To observe the clinically curative effect of acupuncture treatment combined with routine rehabilitation therapy on stroke patients with hemiplegic shoulders.Methods:60 stroke patients with hemiple-gic shoulders were randomly divided into two groups, a treatment group and a control group.30 cases were in each group.Both groups were given routine rehabilitation therapy ( using PNF motion patterns and technolo-gy) .Besides,the treatment group received acupuncture at Jianliao point and Binao point as the extra therapy. The neurologically functional rehabilitation effect was used to evaluate the patients'degree of nervous functional defects.Before and after treatment, the upper limb movement function was evaluated by Fugl-Meyer motor function assessment ( FMA) and activities of daily living ( ADL) assessment in two groups.And visual ana-logue scale ( VAS) assessment was used to evaluate the therapeutic effects of pain.Results:After four weeks’ treatment, the total effective rate was 96.7%in the treatment group and 86.7%in the control group according to neurologically functional rehabilitation effects.Both groups were improved more obviously than before treat-ment by the results of FMA and ADL assessments.The treatment group was much better than the control group (P<0.05).The total effective rate was 90.0%in the treatment group, and 76.7%in the control group by VAS assessment.Conclusion:The acupuncture treatment at Jianliao point and Binao point combined with rou-tine rehabilitation therapy for stroke patients with hemiplegic shoulders is better than the routine rehabilitation therapy only.
