目的 探讨短暂性脑缺血发作患者应用负荷量氯吡格雷联合阿司匹林治疗的临床疗效.方法 随机选取沁阳市中医院于2015年10月—2017年12月收治的120例短暂性脑缺血发作患者为研究对象, 按入院顺序分为对照组与研究组, 每组患者各60例.对照组患者给予阿司匹林进行治疗, 研究组患者在此基础上联合负荷量氯吡格雷进行治疗, 对比分析两组患者的治疗效果.结果 研究组患者治疗后PT、FIB、APTT分别为 (17.03±1.44) s、 (2.05±0.70) g/L、 (39.01±7.58) s, 其中PT和APTT显著升高, 而FIB显著降低, 相较于对照组, 研究组患者的凝血指标明显更优 (P＜0.05);同时研究组治疗后, 总有效率达到95.0％ (57/60), 明显高于对照组的81.7％ (49/60), 差异有统计学意义 (χ2=4.55, P＜0.05), 而研究组出现不良反应的共有6例, 与对照组比较差异无统计学意义 (χ2=0.00, P＞0.05) .结论 负荷量氯吡格雷联合阿司匹林治疗短暂性脑缺血发作患者的疗效显著, 安全性较高, 值得临床继续推广和应用.%Objective To investigate the clinical efficacy of clopidogrel combined with aspirin in patients with transient ischemic attack (TIA).Methods 120 patients with transient ischemic attack admitted to the hospital from October, 2015 to December, 2017 were randomly divided into control group (n=60) and study group (n=60) according to the order of admission. Patients in control groupwere treated with aspirin. Patients in study group were treated with clopidogrel based on aspirin. Comparative analysis of the treatmenteffect of the two groups of patients was done.Results After treatment, PT, FIB and APTT in the study group were (17.03±1.44) s, (2.05±0.70) g/L, (39.01±7.58) s, respectively. PT and APT increased significantly, while FIB decreased significantly. Comparedwith the control group, the coagulation index in the study group was significantly better (P＜0.05). The total effective rate was 95.0％ (57/60), which was significantly higher than 81.7％ (49/60) of the control group. The difference was statistically significant (χ2=4.55, P＜0.05). There were 6 cases of adverse reactions in the study group, and there was no significant difference compared with 7 casesin control group (χ2=0.00, P＞0.05).Conclusion Loading clopidogrel combined with aspirin is effective and safe in the treatment of patients with transient ischemic attack, which is worthy of clinical promotion and application.
