纳米胶原基骨治疗牙周病骨缺损的临床研究

         

摘要

目的:研究牙周骨缺损患者实施修复应用胶原纳米基骨的临床效果。方法:回顾分析我院2008年5月至2011年5月,于我科治疗的115例牙周骨缺损患者临床资料,依据修复方式将其分为研究组(n =85)与对照组(n =30),研究组实施牙周翻瓣胶原纳米基骨植入修复,对照组实施牙周翻瓣瑞福人工骨植入,观察对比两组修复效果。结果:两组胶原纳米基骨及瑞福人工骨植入后,伤口均为一期愈合,均未出现材料外露及伤口感染情况。在治疗完成6个月后,研究组与对照组牙周带的深度与牙龈出血的指数,以及附着丧失情况等均有所改善,同治疗前相较,差异具有统计学意义(P<0.05),但牙龈退缩较前差异无统计学意义(P>0.05)。研究组经治疗后,牙周带的深度与牙龈出血的指数,以及附着丧失情况和牙龈退缩情况,同对照组相较,差异均无统计学意义(P>0.05)。对两组予以 X 线检查,结果显示骨外板清晰并且骨松质的密度增加,达到角形吸收或者垂直吸收位置骨高度可见增加并有新附着出现。患者咀嚼功能恢复,并且牙齿的松动度均降低。结论:牙周骨缺损患者实施修复应用胶原纳米基骨,不仅造价低,同时具备生物活性以及取材简单等优点,能够加快缺损位置重建正常的骨组织结构,并且加快骨缺损进行早期的再生修复,效果显著,应予推广。%Objective: To study the clinical effect on the treatment of bone defect with nano collagen based on periodontal disease .Methods: The clinical data of 115 patients with periodontal bone defect treated in our hospital from May 2008 to May 2011 were retrospectively analyzed .According to the repair method, they were divided into study group (n=85) and control group (n=30).The study group was implanted to re-pair the bone graft and the control group were received periodontal flap redford artificial bone implantation .The repairing effect of two groups were observed and compared .Results:After the nano collagen basal bone and Redford artificial bone implantation , the wound of two groups were primary healing , there were no material ex-posed and wound infection.After 6-month treatment, the depth and the gingival bleeding index and the loss of attachment had improved.The difference was significant (P <0.05), but there was no significant difference in gingival retraction than before (P >0.05).Compared with control group, the periodontal depth and gingival bleeding index with, and loss of attachment and gingival recession of the study group after treatment , were not statistically difference (P >0.05).By X-ray examination of the two groups, the results showed that the bone plate was clear and the density of the cancellous bone was increased , and the height of the bone was increased and there was a new attachment .Patients’chewing function was restored, and the tooth mobility was reduced. Conclusion: The application of collagen nano -based bone in the patients with periodontal bone defect has not only low cost, but also the advantages of biological activity and simple materials .It can speed up the recon-struction of the normal bone tissue structure , and accelerate the early regeneration and repair of bone defects . The effect is significant, and should be popularized.
